Job Description - Team Leader - Social Support Services

Classification: HS3

Salary per hour:  $37.67 per hour + superannuation + generous salary packaging options

Employment Type: Ongoing part-time 64 hours per fortnight

Award: Health and Allied Services, Managers and Administrative Workers (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2021-2025

About you

We are seeking a dynamic individual with a passion for social support services and a commitment to promoting independence and well-being. The ideal candidate will have:

  • Proven leadership skills with the ability to inspire and motivate team members.
  • Experience in planning, delivering, and evaluating social support programs, preferably within the healthcare sector.
  • Knowledge of CHSP, HACC-PYP, and NDIS frameworks and funding requirements.
  • Strong interpersonal and communication skills, with the ability to engage effectively with clients, families, carers, and stakeholders.
  • A person-centred care approach, dedicated to meeting the individual needs and interests of clients.
  • Demonstrated ability to maintain compliance with quality standards, policies, and procedures.
  • A collaborative mindset, with a willingness to work closely with colleagues and external partners to achieve organizational goals.
