Assignment Start Date: 5/8/2024

13 weeks Days 8hour shifts 40 hours guaranteed weekly

The RN is responsible for assessing planning coordinating implementing and documenting care for an assigned caseload of patients in the Home Health department. Develops and implements a home health plan that meets each patients specific needs and is in compliance with regulating agencies federal and state regulations reimbursement guidelines and agency policies and procedures.

DUTIES TASKS AND RESPONSIBILITIES

• Performs accurate and complete assessment of patients; performs initial admission assessment; coordinates and ensures completion of interdisciplinary screenings and assessments; assesses and reassesses pain; assessments are age culture and psychosocial appropriate; recommends appropriate referrals to other disciplines/community resources.
• Formulates a plan of care based on patient needs modifies the plan of care based on patient outcomes; formulates a teaching plan based on identified learning needs and evaluates the effectiveness of learning to include: educating the patient/family regarding treatment plan and providing discharge planning/instructions.
• Makes comprehensive nursing decisions based on the interpretation of facts and evaluation of patient outcomes; participates in care conferences to discuss multidisciplinary team responsibilities patient progress plans for continued care and new problems; utilizes time management skills able to complete care for a full load of patients within scheduled work hours; utilizes pain management techniques; communicate with a physician to obtain/renew/update orders to reflect changes in patient condition.
• Prepares and administers medications based on the age / clinical condition of the patient; starts intravenous lines for medications and fluids; maintains PICC / Central parenteral lines per protocol.
• Assesses the effectiveness of pain medications; revises plan of care as indicated by the patients response to treatment as appropriate.
• Submits updated nursing care plan for the weekend/holiday RN; maintains personal safety while providing care.
• Ensures documentation is complete accurate and timely; communicates documented information for evaluation; completes daily visit sheets.
• Performs all aspects of the position in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors; utilizes established handoff communication protocols.
• Complies with all established safety procedures to ensure a safe environment for patients visitors and staff.
• Participates in performance improvement activities.
• Performs other duties as assigned

EXPERIENCE/EDUCATION/QUALIFICATION

Experience:

• Acute Care Nursing minimum 1 year Preferred
• Home Health Nursing minimum 1 year Preferred

Licenses Certifications and/or Registration:

• Current IL RN License Required
• BLS/BCLS Required
• Valid Drivers license and proof of personal auto insurance Required

EMR System: Strong computer skills with the ability to navigate Cerner EMR programs required

Shift: Days 8hour shifts with weekend oncall rotation every 3 weekends.

Knowledge/Skills/Abilities:

• Excellent communication and customer service skills
• Ability to work independently prioritize and complete tasks promptly
• Knowledge of state federal accreditation and regulatory agency laws and requirements governing home healthcare
