Title: Community Liaison, Older Adults Hub


Reports to: Manager, Family and Community Health


Position Type: Part-Time, Contract (28 hours per week, 3 year contract)


Salary:  $23.52- $26.01 per hour


Vacancy Status: New Vacancy


Location: Kingston


 


Position Summary




As part of the Community Development Team, the Community Liaison Worker will coordinate the Older Adults Hub at the Rideau Heights Community Centre. This staff will be a bridge between the partner agencies offering programming, Kingston Community Health Centres (KCHC) and the City of Kingston.  The Community Liaison will be responsible for the day-to-day operations of the Older Adults Hub providing administrative and program support to all partner agencies.


 


Key Responsibilities 



Community Promotion & Education



  • Support the effective delivery of programming at the older adults hub by being present during activities to assist participants directly, respond to individual needs, and foster a welcoming environment.

  • Participate in outreach to engage communities, participants and community partnerships. Liaise and engage with community partners to support the programs operations and growth.

  • Actively support and promote programs such as the intergenerational program and resources for individuals, groups, community groups and health care networks through events, training, workshops, group programs and meetings.

  • Provide clear, accessible health promotion information for older adults on the benefits of remaining socially and physically active



Program Support



  • Contribute to planning, coordination, and ongoing improvement of programs to ensure they remain responsive and relevant.

  • Assist with the recruitment and retention of a Peer-Led Advisory committee

  • Coordinate and support the utilization of volunteers and student placements throughout the programs offered at the Older Adults Hub.

  • Working with local community partners to advocate for food security programming.

  • In conjunction with Community Development team members organize food programming including a monthly luncheon program.

  • Support the daily setup of group activities, ensuring a welcoming environment with food and refreshments available to encourage participation, engagement, and health promotion.

  • Plan, coordinate, and deliver recurring educational and informational sessions on topics relevant to older adults, with a focus on health, wellbeing, and healthy aging.

  • Coordinate and implement  community engagement initiatives that promote awareness of services and resources available to older adults.



  • In conjunction with manager, work within allocated budget.

  • Participate as a team member in all team functions, program planning,


     team meetings, committees and staff meetings.



Data Management, Reporting and evaluation



  • Collect and enter accurate data for funder reporting

  • Routinely review and compile data for reporting as directed by Manager

  • Complete UCLA3- Loneliness Scale pre and post and WHO-5 Wellbeing surveys.


 


Organizational Responsibilities



  • Comply with all relevant legislation and KCHC policies, including privacy laws

  • Commits to acquiring an understanding of the importance of trauma responsiveness and the impact of Adverse Childhood Experiences (ACEs)  

  • Commits to demonstrating an ongoing commitment to Equity, Diversity, Inclusion, Indigenization and Accessibility (EDIIA) by representing the diverse nature of our communities, promoting and practicing inclusion

  • Supports consistent application and development of KCHC policies and procedures

  • Supports KCHC’s student and volunteer placement programs

  • Promotes awareness of and participation in KCHC activities

  • Demonstrated commitment to continuous learning and quality improvement

  • On occasion, perform other temporary duties as required


 


Basic Education and Experience Requirements



  • College Diploma in social services, community or public health or related field; or 2+ years of relevant experience in community and health promotion

  • Minimum 2 years experience working in a community or health care setting with marginalized older adults

  • Valid Driver’s Licence, Driver’s Abstract, and proof of vehicle liability insurance


 


Knowledge, Skills and Abilities



  • Excellent presentation and communication skills

  • Excellent interpersonal, organizational, advocacy and negotiation skills, including the ability to facilitate meetings

  • Knowledge and experience in best practices in older adults programming

  • Good understanding of ethical principles and boundaries, strong understanding of confidentiality and able to maintain professional relationships at all times with staff and clients

  • Demonstrated ability to work under pressure, to anticipate potential problems/conflicts and take appropriate actions and to meet deadlines

  • Demonstrated ability to work independently and as a team member within an evolving role in a multi-disciplinary environment

  • Strong commitment to health promotion, community development and adult education

  • Must be able to work in a variety of social and physical settings

  • Strong knowledge of Microsoft Office Suite applications


 


Other Requirements



  • Current and satisfactory Criminal and Vulnerable Persons Check

  • Access to reliable vehicle

  • Current CPR and First Aid, an asset

  • Ability to meet the physical demands of the position

  • French language or proficiency in another language, an asset.

  • Ability to work some evening and/or weekend shifts
