Job Description - Clinic Coordinator

Clinic Coordinator Location Victoria, BC :

Cognito Health, established in 2021, is a leading provider of virtual mental health services, dedicated to enhancing the mental well-being of individuals across Canada. Our clinical program is designed to offer evidence-based care for individuals experiencing mild to moderate anxiety, depression, insomnia, and ADHD. Committed to delivering high-quality care and support through innovative, technology-driven solutions, we are currently seeking a passionate and skilled Clinical Coordinator to join our team. As a Clinical Coordinator at Cognito Health, you will play a pivotal role in our mission to make mental health care accessible, affordable, immediate, and integrated.

Responsibilities:

Assessment, Care Planning, Case Management, and Coordination:

  • Proficiently assist in virtual assessments encompassing mental health issues, safety evaluations, and interventions such as patient education, crisis management, and ongoing follow-up care.
  • Coordinate care services, participate in case management, and monitor clinical progress for relevant clients, providing psychoeducation on mental health conditions/medications and navigating external healthcare services. Utilize G Suite spreadsheets to track clients and monitor progress.
  • Maintain accurate and up-to-date electronic medical records, documenting assessments, care plans, interventions, and progress notes in compliance with legal and ethical standards.

Virtual Patient Engagement:

  • Establish rapport with patients through virtual interactions, creating a supportive and therapeutic environment conducive to open communication.
  • Assist clients in navigating the system, particularly during onboarding, utilizing the Cognito Platform, and transitioning out of the program with effective communication under clinical supervision.

Program Development:

  • Provide leadership in developing clinical programs to meet clinical outcomes and organizational goals. Collaborate with key stakeholders on program objectives, structures, and required resources.
  • Collaborate with the medical director to develop and revise policies, procedures, and guidelines, ensuring adherence to evidence-based practice and compliance with relevant regulations and standards.
  • Utilize data analysis to identify trends, measure program effectiveness, and make data-driven decisions for program improvement.

Collaboration:

  • Work closely with relevant clinical and non-clinical teams internally and externally to ensure integrated care delivery and optimal patient outcomes.

Quality Improvement & Assurance:

  • Identify areas for improvement within existing clinical programs and develop strategies to enhance the quality of care.
  • Lead or participate in quality improvement initiatives to optimize program performance.

Support to Clinical Operations:

  • Collaborate with operations leaders and clinical leads to develop relevant content and procedural workflows, and draft communications regarding program changes.
  • Support medical office assistant(s) in managing client communication related to clinical programs, addressing queries, concerns, and complaints while facilitating the exchange of medical information as needed.

Ethical and Legal Compliance:

  • Adhere to all relevant standards, ethical guidelines, and legal regulations governing virtual healthcare services.

Qualifications:

  • RN, LPN, NP or equivalent years experience.
  • Bachelor's degree required
  • Master's degree in nursing (MSN) and/or Certification as a Psychiatric-Mental Health Nurse Practitioner (PMHNP) or Clinical Nurse Specialist (CNS) in psychiatric-mental health is an asset.
  • Minimum of 3 years of clinical coordinating experience, with a focus on mental health.
  • Proficiency in using telehealth technology and electronic health records (EHR).
  • Proficient and compassionate communication skills, including handling challenging conversations.
  • Compassion, empathy, and a strong commitment to patient-centered care.
  • Ability to work collaboratively in a virtual, interdisciplinary team environment.
  • Strong problem-solving abilities and innovative thinking.
  • Exceptional attention to detail and multitasking capabilities.
  • Strong organizational, administrative, and planning skills.
  • Ability to excel under pressure and adapt to changing situations.

Compensation: Starting at $53 per hour
