Job Description - Clinical Nurse Specialist

At Sinai Health, we care, create possibilities and offer hope. We are looking for a  Clinical Nurse Specialist – Geriatrics  to support our Complex Care  program.

Job Outline

We are currently seeking a motivated Clinical Nurse Specialist (CNS) to work collaboratively with the interdisciplinary team to provide comprehensive, high-quality care and services to our patients with a focus on geriatrics and complex conditions. The successful candidate will practice to full scope, using advanced practice skills to provide consultation, assessment, coordination and provision of care to patients with geriatric and complex medical needs. The CNS will assess, plan, implement and evaluate patient needs, support transition planning, patient education and implement and evaluate comprehensive plans of care. In addition to direct advanced patient care, the CNS will take on a role of clinical leadership by engaging in and leading research, quality improvement (QI), and educational initiatives. These responsibilities will be carried out in collaboration with academic and hospital stakeholders, ensuring that the CNS is well-supported and part of a broader professional community. The CNS will provide leadership, mentorship, and education to advance nursing practice and support Learners. Early career CNSs will be provided with formal mentorship to support onboarding and role transition. Summary of Duties:

  • Practices within the full scope in accordance to standards of practice to provide consultation, assessment, coordination and provision of care.
  • Create comprehensive plans of care to support the provision of care with a geriatric focus.
  • Apply knowledge and expertise in dementia care and responsive behaviours to support staff education and care planning.
  • Integrates best practices and evidence into clinical practice; provides exemplary quality care through the application of advanced clinical and theoretical knowledge and skills.
  • Demonstrates sound clinical reasoning in decision making, planning and implementing care.
  • Collaborates with members of the Interprofessional Team in coordinating and providing care for patients.
  • Utilizes skilled communication with patients, families, healthcare providers, and partners, to support coordinated, compassionate care, including Advance Care Planning and goals of care conversations.
  • Uses effective patient/family counseling skills.
  • Employs excellent organizational, critical appraisal and analytical skills.
  • Builds partnerships with community and external stakeholders to support care planning and continuity of care.
  • Continuously identifies areas of improvement and provides leadership for quality improvement initiatives.
  • Provides leadership and promotes excellence in patient-centred nursing care through expert practice, education, consultation and research.
  • Supports education and development of the interprofessional team.

Education & Qualifications:

  • Current College of Nurses of Ontario registration required
  • Master in Nursing (or related field) from a recognized school of graduate studies required
  • 5 years of relevant acute care or post-acute experience in the inpatient setting is highly preferred
  • Additional specialty geriatric education and experience with seniors and frail elderly highly preferred
  • Experience in caring for people with mental health and/or geriatric psychological conditions an asset
  • Demonstrated experience through roles of advanced clinician, educator, researcher and leader
  • Demonstrated excellent oral, written and verbal communication and interpersonal skills
  • Highly developed analytical, problem solving and critical thinking skills
  • Excellent oral, written, and presentation skills, with demonstrated competency in information and communication technology
  • Proven ability to make sound professional judgements and practice decisions, and demonstrate effective time-management and organizational skills in a fast-paced work environment
  • Demonstrated ability to work effectively and collaboratively with others including patients and their families
  • Computer proficiency and/or Intermediate Microsoft Office skills (e.g. Word, Outlook)
  • Current CPR certification
  • Demonstrated satisfactory job performance and attendance history required
