Job Description - COMMUNITY SUPPORT WORKER MENTOR

COMMUNITY SUPPORT WORKER MENTOR

 

CarePartners is extending temporary Mentorship opportunities in support of our Community Support Workers (CSWs). As a result, we are looking for experienced Personal Support Workers (PSWs) to become mentors.

The ‘CSW Mentor – 2024/2025’ role will be a temporary position , and the successful applicant will provide mentorship to newer CSWs in the field. This is a temporary position that will end April.21st, 2024.

 

The CSW Mentor – 2024/2025 will provide CSWs coaching and guidance to help foster and promote the professional development of new employees and is an effective source of advice and encouragement. This mentoring will be performed under the supervision of a Personal Support Manager and schedules will follow the Collective Agreement; availability will need to be consistent with program requirements.

 

 

  • This is a Temporary Full-Time position with guaranteed hours in a bi-weekly period
  • This position will be located in the Oxford County area
  • Schedules will follow the Collective Agreement; availability will need to be consistent with program requirements as follows:

  • YOU SUPPORT CSWs BY:

    • Providing coaching and assistance to CSWs with the following patient care duties:
      • Personal Care
      • Lifts and Transfers
      • Nutrition/Food/Storage
      • Cognitive, Social, and/or Emotional Support
      • Adaptive Aids
      • Home Management
      • Medication
      • Palliative Care; Infant/Childcare
      • Maintaining a Safe Environment
      • Reporting and Communication
      • Meeting with mentees in both the field and the office
      • Is available to the mentee by various means (e.g., email, text, phone, MS Teams, CP Connect)
      • Collecting specific key performance indicators for reporting purposes

 

CarePartners promotes and facilitates, as appropriate, the immunization of employees in order to provide a safe working environment for its employees, to deliver safe care to patients and to fulfill its commitment to evidence-based health promotion. All employees may be required to wear appropriate Personal Protective Equipment as directed by CarePartners and/or mandated by public health authorities.

  • Valid certificate as a Personal Support Worker
  • Minimum 3 years’ experience working as a Personal Support Worker in Community Care
  • Time management skills
  • Computer/mobile technology skills
  • Excellent communication skills, both written and oral
  • Demonstrated acceptable work habits:

    • able to manage competing priorities
    • thoroughness and attention to detail
    • accepts suggestions
    • interacts well with others
    • maintains professional appearance
    • maintains confidentiality of information about patients, caregiver/family etc.

In addition to providing home-based health care, CarePartners also serves the community through clinics, transitional care units, and provides relief in retirement homes and shared care settings. Through our Community Nursing Services outreach program, we’ve been organizing staff-led medical care and clinics in countries with poor access to health care since 2009.
