Department: Aboriginal Health and Wellness Centre of Winnipeg (AHWC) Clinical Operations 
Site: Aboriginal Health & Wellness Centre of Winnipeg, Inc. (AHWC) in the Neeginan Centre 
Address: 181 Higgins Avenue
Reports to: Director of Mobile Healthcare Clinic 
Status: 1.0 FTE (Flexible to hire up to 2 positions in 1.0 FTE) (7.5 hrs/day)
Hours of Work: 11AM-7PM Tuesday – Friday; Saturday 9AM-4:30PM. 
Union: Manitoba Association of Healthcare Professionals (MAHCP) 
Salary: $39.927, $41.273, $42.680, $44.051, $45.324
Date: Immediately – March 31, 2025 (with the possibility of extension)

 

JOB OVERVIEW 

The Mobile Healthcare Clinic (MHC) offers a point of access into the health system specifically for vulnerable and unsheltered community members, who otherwise experience inequitable healthcare access. Travelling and parking near encampments, shelters, and bridges where the need is greatest, offering immediate and ongoing primary care treatment and prevention, is the purpose of the mobile clinic.

Under the general direction of the Director of the Mobile Healthcare Clinic and Clinical Team Leaders, the Crisis Clinician Nurse will utilize their nursing knowledge, skills, and judgment to provide optimal care to patients accessing services. This will include primary care, mental health, and addiction related assessments, interventions, and treatments. The crisis clinician will practice autonomy and will formulate clinical decisions and development of a patient care plan in collaboration with the multidisciplinary AHWC Clinical teams (MHC, RAAM & Primary Care Clinic). Drawing from the strengths of this diverse team and from AHWC’s values and mission statement, this role requires a welcoming, kind, and compassionate person for community members who visit the Indigenous-led Mobile Healthcare Clinic to access supports during their healing journey.

The Crisis Clinician Nurse should demonstrate an understanding of and respect for the histories, traditions, and cultures of Indigenous peoples, inclusive of First Nation, Métis, and Inuit Communities and Nations, including an understanding of the importance of decolonizing healthcare systems and spaces.

 

EDUCATION

• RN/BN, RN Psychiatric Nurse, or RN (AP) 
• Addiction related training (such as The Core Addictions Practices certificate or equivalent education and training experience in substance use / dependency), an asset

 

ESSENTIAL SKILLS 

• Two years related community experience
• Knowledge of Crisis Intervention Theory
• Effective Crisis Intervention Skills 
• Expertise in the area of mental health and addictions
• Knowledge and understanding of psychosocial rehab and recovery
• Knowledge and experience in working with people with substance related concerns
• Proficient at case management
• Excellent Communication and interpersonal skills
• Effective networking and agency relations skills
• Applied Suicide Intervention Skills Training (ASIST)
• Suicide Assessment and Intervention Training
• Trauma Informed Care Training
• Completion of MICST (Manitoba Indigenous Cultural Safety Training)
• Working knowledge of electronic medical records (EMR) preferred

 

LICENSES, REGISTRATIONS

• Responsible for maintaining and providing proof of active registration with the appropriate registering body
• MB Driver’s License 5 (preferred)

 

PHYSICAL AND EMOTIONAL DEMANDS OF THE JOB

• Days, evenings, and weekend shifts 
• Physical ability to perform the duties, responsibilities, and functions of the job
• Must be able to lift up to 30lbs daily
• Must be able to walk up to 5K-7K steps per day and stand for 50 percent of the time
• Must be able to function effectively in a fast-moving environment with frequent interruptions
• Should have good coping skills and stress management

 

MAJOR RESPONSIBILITIES AND DUTIES
(as per AHWC)

• Welcomes and supports all Indigenous - First Nation, Inuit, and Métis – and non-Indigenous community members entering AHWC / Mobile Healthcare Clinic, including people who identify as part of the 2SLGBTQIA+ community and persons who live with disabilities; sees to their immediate comfort
• Nurtures a judgement-free, shame-free environment, focused on harm-reduction and the journey of healing that the individual has defined for themselves 
• Possesses knowledge of social issues such as poverty, homelessness, food insecurity, substance use, mental health/illness, racism, and other challenges faced by Indigenous populations in Winnipeg
• Emphasizes culturally sensitive, culturally relevant, person-centered care
• Delivers safe and clinical practices in accordance with: AHWC’s program standards, policy and procedural operations, and organizational values; Mobile Healthcare Clinic’s program standards; Regional Health and Provincial protocols
• Gives community members hope and encouragement
• Assists in response to emergencies by being familiar with the fire plan, emergency procedures, safety protocols, and location of emergency equipment, etc.
• Responds to emergency codes as per procedures
• Works in collaboration with Director of the Mobile Healthcare Clinic and reports operational issues as advised
• Attends staff meetings as required
• Meets with Director and Medical Director as required
• Adheres to instructions respecting security, use, storage, and destruction of the Personal Health Information (PHI) Act 
• Completes other duties that may be assigned (be a ‘Helper’) when required as part of the RAAM Clinic team, not exceeding stated skills and capabilities

 

MAJOR RESPONSIBILITIES: PRIMARY CARE, CRISIS INTERVENTION and ADDICTION SUPPORT

1. SCREENING
Indicators: 
• Engages client in a manner that builds trust and utilizes a trauma informed lens
• Provides a brief screening function for the purpose of establishing priority for clinical intervention and the appropriate pathway of care within AHWC
• As part of the screening process takes client’s vital signs, and escalates concerning values 
• At all times in working with the client and family, formal or informal supports that are 
present are welcomed and included in the process as appropriate
• Reviews information on client including any existing plans
• Screen for sexually transmitted blood borne infections (STBBI) providing diagnostic testing, 
treatment and management as per physician orders
• Determine immunization status and support vaccination uptake 
• Evaluates potential safety risks and child protection issues and addresses these concerns and a plan of intervention. 
• Refer to appropriate agencies and healthcare system pathways as needed.
• Explores and assesses the resources, psychosocial and environmental stressors that are impacting the crisis situation
• Provides OraQuick HCV Rapid Antibody Testing, if indicated
• Delivers Point of Care HIV testing & treatment, if indicated 

 

2. ASSESSMENT
Indicators:
• Evaluates patients' medical histories and symptoms, potential safety risks and addresses these concerns and a plan of intervention
• Explores and assesses the resources, psychosocial and environmental stressors that are impacting the crisis situation
• Collects complete and accurate data and prioritizes identified needs
• Provide primary care services to patients including diagnose and treat common illnesses and
injuries, specifically wound care
• Coordinate with other medical professionals as part of a larger team.
• Conduct physical exams and patient observations
• Create patient care plans and contribute to existing ones
• Administer and analyze diagnostic tests
• Perform a mental status assessment using the approved tool for crisis services; identifies signs and symptoms of mental or physical illness/distress
• Complete withdrawal assessment tools such as CIWA, COWS etc. to help determine an appropriate treatment path, if applicable
• Complete a Risk Assessment for Suicide using the ASIST model, when appropriate
• Evaluate potential for violence, serious medical complications, victimization, and breakdown of support systems
• Determine the next step in the clinical pathway for the individual within PRIMARY CARE/AHWC / CSU / RAAM

 

3. CLINICAL INTERVENTION 
Indicators:
• Based on the assessment the clinician utilizes the most appropriate clinical intervention
• Interventions are chosen in accordance with evidence-based research 
• Provide crisis intervention to ensure safety, alleviate symptoms, and empower clients;
• Facilitate communication and collaboration among clients(s) and their formal/informal 
supports to promote resolution of the crisis;
• Facilitate effective problem-solving techniques of the client and their formal/informal 
supports toward resolution of the crisis.
• Treat wounds: acute wounds, leg ulcers, and pressure ulcers
• Provide appropriate harm reduction education and training as required (e.g. take home Narcan).

 

4. PLANNING
Indicators:
• The clinician collaboratively develops a plan of intervention with the client and other PRIMARY CARE/AHWC / CSU / RAAM clinical staff and professionals to minimize risk and alleviate symptoms of distress.
• Develop a follow-up plan with the client and support persons that may include referrals to
appropriate resources.
• The clinician identifies existing services and incorporates existing treatment plans in the development of the intervention plan.
• Evaluate the need for accessing emergency services and facilitates timely access.
• Educate clients(s) regarding availability and accessibility of resources.
• Advocate for clients in accessing of resources and ensures the links have been established
• Completes necessary referral and consent forms, including detox forms and establishing connection with Primary Care and other identified supports.

 

5. DOCUMENTATION
• The clinician assumes primary responsibility for ensuring clear and concise reports that detail the nature of the crisis, assessments, the specific interventions and plans for safety and follow-up for each clinic visit, community visit or crisis call.
• EMR documentation complies with the required SOAP format, policies, and legal guidelines

 

6. DECISION MAKING AND ACCOUNTABILITY
The clinician is expected to work collaboratively with other team members. The clinician will be accountable for determining:
• The appropriate action to ensure safety of the client and those around them
• The creating of effective discharge and transition of care planning. Guidelines to be used for decision making may include, but are not limited to, Policy and Procedures, Mental Health Act, Child & Family Services Act, and the Personal Health Information Act.
• PHIA is maintained with the highest standards of integrity 

 

7. HEALTH ASSESSMENTS
The clinician may provide back up for the role of the physician regarding the following tasks:
• Complete and thorough review of patient chart (i.e., health assessment)
• Makes decisions about whether the patient is physically well enough to continue with a physical, mental health, or addiction assessment 
• Consults and liaises with emergency departments, if needed

 

7. COLLABORATIVE CARE
• PRIMARY CARE/ AHWC / CSU / RAAM will be supporting a collaborative model of care to ensure quality care as well as effective utilization of resources. The clinician may at times also provide the same clinical function for individuals that need transportation services in the field, or phone support services.
• Ability to seek out, integrate and value as a partner, the input and ongoing involvement of each patient/client/resident/family/community when designing and implementing care and services
• Ability to differentiate among one’s own role and the roles of other health providers to appropriately establish and achieve patient/client/resident/family/community goals
• Ability to apply the principles of cultural humility, team dynamics, and group processes to enable quality patient care and effective interprofessional collaboration to emerge in everyday practice
• Ability to apply leadership principles that support a collaborative practice model including shared decision-making and accountability for one’s own actions
• Ability to consistently communicate in a respectful, responsive, and responsible person-centered manner
• Ability to engage self and others, including the patient/client/resident/family/community in a positive manner to constructively address disagreements as they arise
• Ability to engage in relationships with care, dignity and respect regardless of race, ethnicity, culture, ability or language proficiency
• Ability to collaborate as required with withdrawal management services and addiction treatment agencies
• Adheres to all safety and health regulations and safe work practices
• May be required to perform other duties and functions related to this job description not exceeding above stated skills and capabilities.

 

CROSS TRAINING

• The Clinician may be cross trained in AHWC’s other clinics including RAAM and Primary Care Clinics to support temporary coverage of shifts when staffing is constrained.
