COMPENSATION: $52,535 - $63,260 Annually Prorated

CONTRACT DURATION: 8 Months - Ending December 31 st , 2023.

To help fulfill its mission, LOFT is hiring a Case Manager.

 

 

JOB SUMMARY:

The Case Manager on the M-DOT Hotel Project Team provides direct service to homeless individuals with mental health, health and addictions issues. The position provides both site-based services and community based assessment and case management the group of complex clients who are living in physical distancing hotels. Case Managers work as part of a team that also includes a Team Leader and Peer Supporters, to meet clients’ housing and other social and health needs.  The program works closely with nurses and physicians from Inner City health Associates, to assess, plan, treat and support clients. The position will involve driving and a valid Ontario driver’s license is required. Access to a car is a requirement.

 

Note The Case Manager is expected to meet clients outside of the office. Visits are provided in the clients’ homes or other locations in the community, which requires physical activities such as significant driving, frequent stops, getting in and out of the car and climbing stairs. Accompanying clients to appointments is an expectation of the position.

 

 

WHAT YOU WILL DO:

Direct Service Responsibilities:

  • Engages effectively with individuals experiencing serious mental health issues and/or addictions, cognitive difficulties, brain injury or primary health care issues
  • Conducts outreach shifts with other team members as scheduled
  • Develops relationships with individuals served by the team and addresses basic and immediate needs
  • Engages in crisis intervention/prevention with clients
  • Case manages from a psycho-social rehabilitation and recovery model,  linking clients with other supports as necessary, including housing, transportation, shelter placement, identification (ID) services and translation services, among others
  • Liaises with Ontario Works (OW) and the Ontario Disability Support Program (ODSP) as required
  • Completes housing applications for M-DOT clients and act as an advocate to place clients in appropriate, affordable housing
  • Works from a harm reduction approach with clients dealing with addiction issues
  • Advocates on behalf of homeless individuals in accessing a range of services
  • Works cooperatively with team members, hospital personnel, other service providers and City staff in order to better meet client’s needs
  • Facilitates transfer of cases to Streets to Homes funded follow-up support services or ongoing case management services

 

Indirect Service Responsibilities:

  • Documents client-related activities accurately and in a timely fashion on the Client Information System’s database
  • Actively participates in regular supervision, case reviews, team meetings and training sessions
  • Submits time sheets, expense claims and other required information in accordance with agency regulations
  • Represents the agency at external workshops, inter-agency initiatives, staff training and other community-based activities
  • Shares the driving responsibilities with other team members
  • Other duties as assigned by the Program Manager or the Executive Director

 


WHAT YOU BRING:

Must Haves

  • A degree/diploma in health care/social services or other related field with a minimum of three years relevant experience in the mental health sector
  • Knowledge of psychosocial rehabilitation principles and practices, including an understanding of the philosophy of recovery
  • Excellent interpersonal skills, sound knowledge of supportive counselling skills and the ability to form relationships with marginalized individuals
  • Knowledge of community resources for housing, including supportive housing and shelters
  • Knowledge of concurrent disorders and experience with the continuum of addiction services
  • Knowledge of community resources for mental health and substance use
  • Ability to work closely with teammates and medical professionals and participate effectively as part of the larger multi-disciplinary team
  • A valid Ontario driver’s license is a requirement for the position
