About the Opportunity

Reporting to the Health Service Manager of Palliative Care, or designate, the incumbent is a member of the Palliative Care Team and participates in the planning, development, and provision of Social Work services to ensure psycho-social care is core to the care of individuals living with or dying from a life-limiting illness and their family members. The Social Worker works to full scope of practice as an independent practitioner within the team. This includes supporting the development and integration of palliative care services within health care and the community and providing collaborative, client centered care across the continuum, supporting the psychosocial and emotional well-being of individuals (includes patients, families, consultation team, groups, populations, and communities). This position includes considerable collaboration within the team and with community partners.

 

The Social Worker ensures that the principles and practices that guide NSH such as the mission, vision, values, expected behaviors, and the leadership philosophy along with organizational health, population health, ethics, safety, quality, partnerships and interprofessional collaboration are integrated within the services one provides and through the messages one delivers.

About You

We would love to hear from you if you have the following:

  • Masters of Social Work required
  • Certificate of Registration and Membership in the Nova Scotia Association of Social Workers and Canadian Association of Social Workers.
  • Graduate of a recognized Palliative Care course, or willing to enroll in a recognized program within one year of hire and, successful completion of non-violent crisis intervention training.
  • Minimum five years clinical experience
  • Palliative Care or Oncology experience preferred
  • Strong counselling and communication skills
  • Capacity to achieve a high degree of autonomous practice
  • Ability to work within a multidisciplinary team context
  • Knowledge of issues related to family violence, sexual abuse, addictions, grief and loss, end of life, disability, crisis intervention, caregiving and continuum of care and intervention skills that come from knowledge of theoretical frameworks/approaches to practice.
  • Demonstrated experience facilitating educational programs and family and team conferences.
  • Competency in basic computer skills (Microsoft Suite, E-mail, Internet, Excel, Meditech, SEAscape).
  • Competencies in other languages an asset, French preferred.
  • Physical capabilities to perform the duties of the position such as dealing with stressful situations, traveling between sites within the geographical area of the Annapolis Valley and to meetings in other localities as required.

  • Permanent, Full-time / 1.0 FTE / 75 Hours Bi-weekly 

Compensation and Incentives

$37.31 - $49.05 Hourly
