Salary range

The salary range for this position is CAD $31.56 - $33.28 / hour

Detailed Overview

Under the general supervision of the Coordinator or designate, the Health Care Worker works closely with the interprofessional community mental health team in the provision of time limited services to adults with severe and persistent mental illness who require support in the community; provides services that focus on stabilizing, bridging and transferring the client(s) to appropriate services; provides escort services; works in close association with multidisciplinary community mental health staff, agencies and community providers; assists in the evaluation of process and outcomes to ensure the continuing quality of the services.

Responsibilities
  1. Participates in case planning with clients and/or case manager by performing duties such as interviewing clients to identify problems and needs, obtaining required information from a variety of sources such as families and care providers, documenting client needs and wants, and assisting clients to determine appropriate programs.
  2. Communicates and consults with the coordinator, case manager and/or other mental health team members about client care issues, resources and discharge planning; explains plan of care to clients and their families; refers clients to rehabilitation and other adjunctive services.
  3. Provides client advocacy services with other agencies, services and service providers.
  4. Accompanies clients to doctors' visits and other scheduled appointments.
  5. Observes and monitors clients' progress; communicates and consults regarding the care and support plan with the client and the mental health care team in order to ensure continuity of appropriate care for the client.
  6. Provides supportive counselling/demonstrations/modelling/behavior management, as defined by the clinical team and care plan, such as: facilitating clients' attendance at rehabilitation, social, and vocational training programs; supporting clients at interviews with human resources, commercial sources and community colleges; enabling clients to find therapeutic volunteer placements and by facilitating clients' attendance at community recreational activities.
  7. Acts as a resource to the client's family or the mental health team regarding the availability and suitability of support services to ensure the development and implementation of an appropriate care plan; facilitates the involvement of clients in self-help approaches and client initiated changes to the mental health service system.
  8. Assists in maintaining client records, in accordance with established standards, policies and procedures; writes timely and accurate reports of relevant observations including client and family teaching and evaluation of care to ensure continuity of communication and client documentation.
  9. Maintains related reports, records, and statistical data, in accordance with established standards, policies and procedures.
  10. Participates in the evaluation of client care programs, techniques, standards, policies and procedures by providing feedback about effectiveness and client care outcomes.
  11. Participates in the orientation of students
  12. Participates in multidisciplinary conferences and with psychiatrist as required.
  13. Performs other related duties as assigned.
Qualifications

Education and Experience

Grade twelve (12) plus successful completion of the Community Mental Health Worker Certificate or Social Services Worker Diploma and two (2) years' recent related experience, or an equivalent combination of education, training and experience.

Valid Class V BC Driver's License and access to a personal vehicle for business related purposes.
