Position Description

POSITION SUMMARY The Senior Manager, Post-Acute Care & Community Settings is responsible for overseeing the bedded programs within the Post-Acute Care portfolio (approximately 200 across 3 locations), leading initiatives with congregate setting partners and the Greater Hamilton Health Network (inclusive of but not limited to pandemic responses), and developing new programs and/or services for seniors and patients with complex health conditions (within SJHH & with external partners). This oversight includes operational activities such as budget and human resources management; developing program specific strategic goals, such as volumes and capital planning, that are in alignment with the corporate strategic directions and vision; and providing leadership to all members of the program team. As a team leader, the Senior Manager ensures that all health care professionals within his/her program function within the Standards of Practice as defined by the appropriate regulatory body. The Senior Manager must also collaborate with the medical co-lead(s) to advance the program's clinical education, service and research opportunities. Another key role of the SM is to liaise with other community providers, provincial agencies, funders (MOH, OHW, etc.) and key external stakeholders.

QUALIFICATIONS
  • Undergraduate Degree in regulated health professional discipline
  • Master's Degree in a related discipline, health sciences or MBA
  • Member in good standing with the Regulated Health Professional College and Provincial Professional Association
  • 5 to 7 years providing practical and related experience
  • Experience in an academic health science centre
  • Progressive leadership or management experience in a healthcare organization
  • Demonstrated continued education in Leadership, Budgeting, Performance Management, Labour Relations, Coaching, Team Building, Research Skills, Teaching Skills
RESPONSIBILITIES
  • Manages the development, implementation, evaluation and maintenance of quality improvement initiatives which are in alignment with established SJHH standards.
  • Analyzes appropriate internal and external data and information to identify opportunities in collaboration with stakeholders for improving services and patient outcomes in accordance with MOH, MOLTC, Rehabilitative Care Alliance and Accreditation Canada standards.
  • Directs program practices and processes to ensure excellence in patient care delivery in accordance with MOH, Rehabilitative Care Alliance, Accreditation Canada and other legislative and regulatory standards.
  • Reviews and revises policies and procedures that will ensure excellent delivery of clinical service and effective program operations.
  • Collaboratively designs and implements methods and techniques to monitor, track and continuously improve the delivery of service.
  • Articulates the performance expectations of all direct reports
  • Determines the staffing requirements, including nursing skill mix and manages the recruitment, selection, and retention of all staff within the approved budget guidelines.
  • Develops, mentors and coaches staff to maximize potential
  • Fosters and ensures effective employee and labour relations through knowledgeable interpretation and implementation of hospital policies and collective agreements
  • Acts as a coach, mentor and role model to team members and provides leadership to all staff by providing an environment where shared governance, empowerment and respect for diversity can grow
  • Communicates and co-ordinates with the Senior Director, and Human Resources and other departments as required
  • Oversees the annual operating budget for all bedded programs for the utilization of resources.
  • Collaborates with Clinical Managers to manage actual expenditures to ensure it remains within approved budget guidelines
  • Participates in the development of the operating plan in consultation with stakeholders such as support services and departments, physicians, staff, consumers and community partners.
  • Utilizes allocated resources (staff, supplies, space and equipment) to achieve the programs and organization's vision, goals and service objectives
  • Contributes to corporate efforts and initiatives, as appropriate, to enhance SJHH's mission, values and goals.
  • Represents the portfolio internally on SJHH committees, as required, and SJHH in the external community at conferences, workshops, etc.
