POSITION SUMMARY The Senior Manager, Post-Acute Care & Community Settings is responsible for overseeing the bedded programs within the Post-Acute Care portfolio (approximately 200 across 3 locations), leading initiatives with congregate setting partners and the Greater Hamilton Health Network (inclusive of but not limited to pandemic responses), and developing new programs and/or services for seniors and patients with complex health conditions (within SJHH & with external partners). This oversight includes operational activities such as budget and human resources management; developing program specific strategic goals, such as volumes and capital planning, that are in alignment with the corporate strategic directions and vision; and providing leadership to all members of the program team. As a team leader, the Senior Manager ensures that all health care professionals within his/her program function within the Standards of Practice as defined by the appropriate regulatory body. The Senior Manager must also collaborate with the medical co-lead(s) to advance the program's clinical education, service and research opportunities. Another key role of the SM is to liaise with other community providers, provincial agencies, funders (MOH, OHW, etc.) and key external stakeholders.
