JOB SUMMARY: The Diabetic Footcare RPN reports to and works under the direction and supervision of the Clinical Lead Supervisor, Primary Care Services, Six Nations of the Grand River Elected Council.

The purpose of the Diabetic Foot Care RPN position is to provide safe and quality foot care services to diabetic or high-risk diabetic clients within the scope of practice of a Registered Practical Nurse of the College of Nurses of Ontario. Daily functions are in accordance with guidelines as outlined by the Diabetes Wellness Program, Six Nations Department of Well-Being and Six Nations of the Grand River Elected Council.

To improve access to foot care services, self-management, education and prevention services for people living with diabetes or at high risk of developing diabetes.

Reinforce the values and philosophy of the Diabetes Wellness Program in all aspects of the position upon initial contact with the clients.

BASIC QUALIFICATIONS:

• Registered Practical Nurse with a Diploma in Practical Nursing and current registration with the College of Nurses of Ontario, with a license in good standing.


• Current First Aid and Cardiopulmonary Resuscitation (CPR) certification is required.


• Willingness to participate in cultural safety training is required.


• Demonstrated skills and training in foot care and diabetes education are required.


• Immunization certification, completion of a Community Health Nursing program, and computer training are considered assets.

REPORTING RELATIONSHIP:

KEY DUTIES & RESPONSIBILITIES:

Technical Functions

• Plan and implement culturally relevant diabetic foot care health programs within the Diabetic Wellness Program


• Employs appropriate nursing skills in the delivery of service to clients such as assessment skills, use of nursing process in preparing plans of care for clients, manual dexterity in performing nursing procedures and excellent communication skills re:
  
  
  
  • interpersonal relations, family and individual health teaching according to The College of Nurses of Ontario standards of nursing practice and ethical guidelines.
  
  
  
  


• Execute additional duties as assigned by the Clinical Lead Supervisor.

Administrative Functions

• Maintains and manages clinic operations.


• Projects, workplace activities, monitors budget flow, month-end, quarterly reports and year- end.

Communications Functions

• Collaborates as necessary with appropriate health care providers and community agencies.


• Utilizes/refers to community agencies as required.


• Attends staff and program meetings as required.


• Liaison with other departments within the Department of Wellbeing and outside agencies.

Other Functions

• Will be required to perform other job-related duties as may reasonably be required by the Clinical Lead Supervisor.


• Provides support to the Six Nations Emergency Measures Plan by ensuring awareness of the Emergency Measures Plan and assistance as instructed by the Director of Wellbeing.


• It is the responsibility of the employee to be aware of and adhere to all policies and procedures, including those that relate to client safety, staff safety and risk management.

WORKING CONDITIONS:

Work requires physical activity and mental stress; required to work inside/outside; requires travel; requires extensive interaction with the public who at times may be hostile or irate; subject to interruptions, deadlines, unscheduled hours.

Work requires the ability to prioritize tasks, work independently with minimal supervision, and cope with many demands and time constraints.

Advocates on behalf of clients.

With External Agencies

Represents and promotes the Diabetes Wellness Program clinic interests relative to the Department of Well-Being, maintains awareness of legislative policy and program changes; seeks to develop sound, professional working relationships.

KNOWLEDGE AND SKILLS:
