Cardiac Rehab Nurse Health Educator

The Prince Edward Family Health Team (PEFHT) is seeking a Cardiac Rehab Nurse Health Educator (3 days per week) to join our team serving the residents of Prince Edward County and those of surrounding communities. 

PEFHT provides a single point of access to health care services for all County residents.  Our interdisciplinary care team offers comprehensive patient-centred, primary health care including disease diagnosis, treatment and management, prevention and cure, rehabilitation, palliative care, and community health promotion.

Within the context of a patient-centred care model, and working within a multidisciplinary, inter-organizational team, the Cardiac Rehab Nurse will:

• Function as an educational resource for patients, families, and team members;


• Assist in, coordinate, and conduct patient, client, and family education as it relates to lifestyle change to enhance cardiovascular health;
  Assess presence and extent of modifiable cardiovascular disease (CVD) risk factors;


• Assess cardiovascular status and clinical symptoms;


• Develop a CR and CVD risk reduction care plan with patient;


• Monitor patient progress regularly and assess achievement of CVD risk reduction goals; readjust care plan as required;


• Liaise with the patient’s other care providers to optimize CVD risk factor reduction;


• Maintain effective communication with all team members;


• Participate in multidisciplinary team meetings regarding planning, implementation and revision of patient care programs;


• Optimize patient safety;


• Monitor physiological responses to exercise (e. g. HR, BP and arrhythmias), assess participants for signs and symptoms of cardiovascular distress, and initiate appropriate course of action and follow up;


• Implement emergency protocols and direct/assist in patient management;


• Communicate medical concerns or change in patient status to medical director and patient’s primary care provider;


• Maintain clear and concise patient care records.

Basic Qualifications:

• Graduate with Diploma or Degree in Nursing


• Current Registration with the College of Nurses of Ontario


• Current membership with RNAO


• Additional training specific to acute management of cardiovascular symptoms and events


• Excellent verbal and written communication skills


• Certification:  ACLS, BCLS for Healthcare Providers with AED training.

Assets:

• Training:  Additional training specific to CR and CVD prevention, additional training specific to Cardiac Rehab (PEFHT will work in collaboration with Providence Care Cardiac Rehabilitation Centre, Kingston to provide additional CR training);


• Certification:  CCN Cardiovascular Nursing certification;


• Experience:  2 years’ experience with cardiac patients.
