Job Description - Care Manager

About the Role

We are looking for a compassionate and highly organized Care Manager to oversee and coordinate care services for individuals in healthcare, residential, or community settings. The Care Manager will be responsible for developing care plans, managing staff, ensuring compliance with regulations, and delivering high-quality support tailored to each individuals needs. This role requires strong leadership, empathy, and a deep commitment to improving the quality of life for clients.

Key Responsibilities

  • Assess client needs and develop personalized care plans in collaboration with healthcare professionals and families.

  • Supervise, train, and support care staff to ensure high standards of service delivery.

  • Monitor and evaluate the effectiveness of care plans and adjust as necessary.

  • Ensure compliance with healthcare, safety, and safeguarding regulations.

  • Serve as the primary point of contact for clients, families, and external agencies.

  • Manage schedules, staffing levels, and daily operations within the care setting.

  • Maintain accurate documentation, reports, and care records.

  • Promote a culture of dignity, respect, and person-centered care.

  • Handle complaints, resolve issues, and implement improvements in care delivery.

Qualifications & Skills

  • Proven experience as a Care Manager, Care Coordinator, or in a senior healthcare role.

  • Strong leadership, organizational, and problem-solving skills.

  • Excellent communication and interpersonal abilities.

  • In-depth knowledge of health and social care regulations, safeguarding, and best practices.

  • Ability to handle sensitive situations with empathy and professionalism.

  • Proficiency in care management systems and record-keeping.

Education & Experience

  • Bachelors degree in Nursing, Healthcare Management, Social Work, or a related field.

  • NVQ Level 4/5 in Health and Social Care or equivalent qualification preferred.

  • Minimum 35 years of experience in healthcare or social care, with at least 2 years in a supervisory or management role.
