Job Description - Case Manager

Jean Tweed is a leading, community-based organization that provides a supportive environment for women with substance use, mental health, and gambling concerns. Recognizing the diversity of women’s experience in today’s society, our programs seek to address a range of issues in a woman’s life that may contribute to substance use and gambling concerns.

The Jean Tweed Centre is building capacity to transform the level of support for women with substance use, mental health, and gambling concerns.

How will you help to get us there?

As the Case Manager, you will:

  • Lead a collaborative wrap-around process to ensure comprehensive service coordination that reflects individual needs.
  • Provide case management support, including counselling, outreach, on-going assessment, goal planning, referral, development of support systems, system navigation, etc.
  • Provide referrals and support to access health and community-based services.
  • Complete required case notes, evaluations, and data entry reports in an accurate and timely manner.
  • Conduct job responsibilities with a commitment to harm reduction, trauma- informed, women-centered and anti-oppressive practices.

Pay: CA$29.70 - CA$34.75 per hour

Hours of Work: The hours of work are 9am-5:00pm. 

What will you bring?

  • Bachelors Degree in Social Work, or a related discipline.
  • A combination of 3 years of work experience in the field of Mental Health, Substance Use, and/or related field.
  • Expertise in providing counselling and case management to individuals who are experiencing or have experienced a complex history of mental health concerns, substance use, and/or homelessness.
  • Working knowledge of best practices in mental health, substance use, harm reduction, trauma and the therapeutic relationship.
  • Working knowledge of trauma-informed practice, women-centred principles and best practices for women and substance use.
  • Working knowledge of housing, health care and community resources in the Toronto area and related provincial resources.
  • Ability to work independently with minimal day-to-day supervision.
  • Crisis intervention skills, with well-developed problem-solving abilities and interpersonal skills.
