Purpose of the Position: Community Homes for Opportunity (CHO) assists individuals over the age of 18 living with serious mental illness by providing stable, affordable, appropriate housing and support services. CHO aims to support individuals to achieve and maintain physical, mental health, and wellbeing, as well as foster independence and enhance participation and integration into the community of their choice. CHO is a person-centered, flexible service, reflecting the individual and their changing needs. The Housing Case Manager will collaborate with home operators and tenants of four (4) congregate living homes to support the best possible living environments. They will work to support referrals to congregate living homes, support home operators with operations, and facilitate tenant access to resources and supports to maximize their independence and potential and promote their recovery.
Education and Qualifications:
Salary scale: $29.85 to $39.09 per hour commensurate with qualifications and experience
Benefits include Comprehensive benefit plan and enrollment in a Registered Retirement plan with HOOPP where contributions start on date of hire.
