The Chinese Client Intervention Worker will be required to work 21 hours a week; Wednesday, Thursday and Friday from 8:30 AM to 4:30 PM.
Position Summary
Reporting to the Manager, Comprehensive Intake and Care, the Client Intervention Worker will support Chinese seniors, Chinese Adults with disabilities or chronic illnesses in the community requiring case management assistance and support to improve their quality of life.
Duties and Responsibilities
Individual Case Management
Offer case management services to seniors and their family members who require tangible assistance, emotional support, counseling and referral
Advocate for seniors who are vulnerable and receive unfair treatment, to work together with them for solutions.
Develop assessment and develop working plans together with the seniors, make necessary intra and inter agency referrals.
Collaborate with other human service organizations to promote the quality of independent living of the seniors.
Maintain and update service statistics, to document service needs and service gaps where appropriate.
Conduct initial and annual interRAI CHA assessments.
Administration
Maintain appropriate documentation for statistics and files.
Provide information for funder reports as necessary.
Maintain client files.
Organizational Responsibilities:
Work collaboratively with seniors department staff, other Dixon Hall staff and departments and volunteers
Operate in an effective, efficient and client focused manner to achieve the best outcomes for clients
Actively participate in department and agency meetings and events
Work under the guidance of the agency and program policies and procedures, including the Mission, Vision and Values of Dixon Hall and the agency’s strategic direction.
Meet regularly with manager to discuss individual, team and the overall effectiveness of client services
Undertake other tasks and responsibilities as assigned within the purview of the job description
Other duties as required.
Qualifications
B.S.W., gerontology or related training and experience
2-3 years' experience working with seniors or in the social services sector
Case management experience with vulnerable or high risk populations including seniors, adults with disabilities, people with mental health problems
Demonstrate interpersonal skills
Experience working with volunteer coordination and development
Excellent knowledge of issues affecting seniors in downtown east Toronto
Excellent verbal and written skills
Experience with conducting interRAI CHA assessments an asset
Ability to write and speak Cantonese and/or Mandarin a requirement
Must be qualified to work in Canada legally for the employer.
Able to provide a Vulnerable Sector Screening check with a ‘Clear’ or satisfactory result that meets the operational requirements of the Agency.
Must have up-to-date vaccinations against COVID-19 with a vaccine that has been approved for use in Canada.
