The Service Planning Coordinator (SPC) works in partnership with the Children’s Treatment Network to support children and youth with multiple or complex special needs, along with their families. Acting as the primary point of contact, the SPC supports children and youth under the age of 18, as well as young people aged 18 to 21 who are still attending school.
The SPC will lead the development, monitoring, and evaluation of the Coordinated Service Plan in collaboration with the child, youth, family, and service providers from across multiple sectors.
This position promotes a collaborative, inter-agency approach to care and helps strengthen internal and external team relationships by fostering connections within the Children’s Treatment Network system.
