Weekdays: Monday to Friday
Hourly Rate of Pay: $45.00 to $63.00

Carecor is seeking a dedicated Diabetes Nurse to support the Mobile Diabetes Clinic in delivering accessible, culturally safe diabetes care to Northern First Nations communities. In this role, you’ll work collaboratively with our healthcare partners to provide assessments, complication screening, education, and follow‑up through mobile and telemedicine services. Guided by Carecor’s commitment to compassionate, client‑centered care, you will help improve wellness and diabetes outcomes for individuals who may otherwise face barriers to accessing essential health services.

Key Responsibilities

• Advocate for First Nations clients affected by diabetes and related health conditions.


• Plan, deliver, and evaluate diabetes care across the lifespan, providing education, screening, testing, and follow‑up in clinic, community, and home settings.


• Review medical histories, maintain accurate EMR documentation, and use data to inform care planning.


• Provide individual and group diabetes education using culturally appropriate and diverse learning approaches.


• Collaborate and communicate with primary care providers, mental health teams, and community partners to coordinate care and share significant findings.


• Act as a resource to staff and community members, promoting diabetes awareness through training, events, and outreach.


• Compile program data, support quality improvement, and follow Diabetes Canada Clinical Practice Guidelines and professional practice standards.


• Participate in ongoing learning and professional development; perform additional duties as assigned.

Job Qualifications

• Knowledge of First Nations culture (Carrier and Sekani preferred) and diabetes management.


• Strong communication, organization, teaching, and documentation skills.


• Able to work independently and within a multidisciplinary team.


• Proficient with Microsoft Office, EMRs, and common office equipment.


• Registered RN, LPN, or RD with current registration; Diabetes Educator certification or willingness to obtain.


• Experience in clinical care or cross‑cultural health promotion.
