KCR Community Resources is seeking a dedicated and compassionate Family Outreach Worker to join our team in a full-time position. In this role, you will provide family-centered support to pregnant individuals and families parenting children from birth to age five. The Family Outreach Worker works closely with families to assess needs, identify strengths, set goals, and provide individualized home visits. This position supports healthy pregnancy outcomes, positive parent-child relationships, child development, parenting skills, and family well-being.
The role also includes connecting families with community resources, making referrals, maintaining accurate documentation, and supporting program evaluation. This is a meaningful opportunity to make a positive impact in the lives of families and young children in our community.

Prime Functions:
Facilitating and delivering effective services
Assess families’ needs, identify and develop their strengths, integrate information, and establish goals
Schedule and conduct home visits with families and their children according to program model
Adhere to Health and Safety Policies and Procedures for home visits
Administer assessment tools to identify participant needs for goal setting
Provide parents with current “best practice” resources, including normal child growth and fetal development, mental health, and community supports
Liaise with community supports and make referrals as required
Recognize signs and symptoms of families and individuals at risk for adverse child or family outcomes
Fostering the development of others while providing Participant centered services
Establish healthy relationships using a family centered approach; focus on strengths and parent/child attachment
Establish and review/update goals semi-annually to assist families with moving towards successful outcomes
Assist pregnant women to access regular prenatal care, provide information/support that promotes optimal physical and mental health, including harm reduction strategies
Teach positive parenting skills and provide support to help parents nurture child development, address safety issues, and manage common childhood concerns
Model playtime with children during home visits, including outdoor play
Collect and compile program data in order to evaluate program effectiveness and impact
Complete charting in accordance with program policy after family visits/contacts
Collect and document relevant data to assist in compiling program and performance measurement outcomes
Practice with awareness of legislation relating to child protection, reporting, confidentiality and consent
Qualifications Required:
Minimum post-secondary education or equivalent experience in a related field with preference in early childhood, education, health or social sciences. Additional training and relevant experience in counseling and family development is an asset
Experience working with culturally diverse, overburdened families with young children and with other community agencies
Able to work independently and as part of a team
Excellent time management and organizational skills; committed to on-going professional development
Good communication skills; written and verbal
Ability to establish therapeutic relationships
Demonstrated computer proficiency
Skills in applying adult learning principals
Personally warm, genuine, compassionate, encouraging, hopeful, non-judgmental, flexible and adaptable, respectful of client hopes and desires
Practices self-care to manage stress and prevent burnout
Possession of a valid British Columbia driver’s license and vehicle for work
