The Role: Family Physician in Toronto

• Provide comprehensive primary care with a focus on prevention, chronic disease management, and patient education.
• Collaborate in an interdisciplinary team to deliver patient-centered, team-based care.
• Build a patient panel focused on prevention and longevity, emphasizing proactive and personalized care.
• Leverage technology and innovation to enhance patient care, including POCUS, mental health technologies, and genetics.

Experience You Bring & Qualifications

• Certification in Family Medicine (CCFP) from the College of Family Physicians of Canada (CFPC)
• Current registration with College of Physicians and Surgeons of Ontario (CPSO)
• At least three of clinical, primary care experience
