Position Overview

The Rehabilitation & Geriatrics Program is dedicated to providing access to a range of services needed by patients focusing on their individual requirements.  The team ensures that access to the full continuum of resources is made available at the point of contact and are available throughout the program.  It is the goal of the Rehabilitation & Geriatrics Team to maintain a seamless continuum of care and to implement a model for coordinated rehabilitation services within the City of Winnipeg, and in support of rural Manitoba and other RHAs.


The Geriatric Rehabilitation Clinician contributes to the care of high-risk, frail adults aged 65 and older through the application of comprehensive, multidimensional assessment, advanced clinical reasoning, care planning, coordination and evaluation across hospital, rehabilitation, urgent care, emergency and community settings. Clinicians support timely and appropriate access to geriatric rehabilitation services by assessing functional status, medical complexity, cognitive and psychosocial factors, and environmental considerations. They provide recommendations and advocacy to optimize rehabilitation potential, safe transitions, and system flow. 


In patient and acute care settings, the Clinician facilitates patient flow by identifying older adults appropriate for geriatric rehabilitation, supporting transfers from inpatient units, emergency/urgent care and contributing to discharge and transition planning.


In community and consultative settings, the Clinician conducts in-home and outpatient Assessments, provides geriatric consultation to emergency/urgent care, and supports direct admonition to inpatient rehabilitation when indicated. When functioning as part of the Geriatric Program Assessment team (GPAT), the Clinician works collaboratively with an interprofessional team to evaluate client strengths and needs, develop client-focused solutions, and provide recommendations to primary care providers and community services. 

Experience


  • Minimum of three (3) years of current clinical experience in clinical geriatrics.

  • Ability to work with minimal supervision as well as collaboratively in a team.

Education (Degree/Diploma/Certificate)


  • Entry-to-practice academic credential in a relevant health care profession from an approved Canadian university program or the approved equivalent if internationally educated.  Relevant health care related Masters degree preferred.

  • Must successfully complete individualized self-directed geriatric training provided through the program to each clinician in order to independently complete a comprehensive geriatric assessment.  

Certification/Licensure/Registration


  • Current registration/licensure with a Manitoba Health care professional licensing body (CRNM, COTM, CPM, MCSW).

  • Valid Manitoba driver’s license and access to a vehicle required.

Qualifications and Skills


  • Ability to work with minimal supervision as well as collaboratively in a team

  • Self – directed

  • A demonstrated interest in geriatrics

  • Ability to establish good interpersonal relationships with clients and health care professionals

  • Effective written and verbal communication skills

This position requires a current satisfactory Criminal Records Check (including Vulnerable Sector Search), Child Abuse Registry Check and Adult Abuse Registry Check as conditions of employment. The successful candidate will be responsible for any service charges incurred. A security check is considered current if it was obtained no more than six (6) months prior to the start of employment.

The WRHA legal entity includes Churchill Health Centre, Deer Lodge Centre, Golden Door Geriatric Centre, Golden West Centennial Lodge, Grace Hospital, Middlechurch Home of Winnipeg, Pan Am Clinic, River Park Gardens, Victoria General Hospital, WRHA corporate programs, and WRHA community health services.
