Reporting to the Supervisor, Home and Community Care, the Home and Community Care II The is a cornerstone of Nunavut’s health care system, ensuring that vulnerable clients receive safe, dignified, and culturally appropriate care in their homes. This position provides essential personal care and specialized support, including assistance with bathing, grooming, mobility, medication reminders, and individualized care for palliative, rehabilitation, and post-acute clients. 

Main responsibilities for the Home and Community Care II include: 

• Providing comprehensive personal care, including bathing, grooming, dressing, toileting, medication reminders, and mobility assistance, ensuring client safety 

• Performing specialized care for palliative, rehabilitation, and post-acute clients, applying individualized care plans and client-specific training to meet complex health needs

• Monitoring client health status and living conditions, exercising independent judgment to identify risks and determine when urgent escalation to health professionals 

• Preparing and planning meals, including therapeutic diets, and assists with grocery shopping to support nutritional health and recovery

• Maintaining accurate and timely documentation of care activities, observations, and client interactions to ensure continuity of care and compliance with program standards

• Advocating for client needs within the health system, applying a cultural lens to ensure care plans respect Inuit values and traditions

The knowledge, skills, and abilities required for this job are usually obtained through completion of grade 10 education and two years of home care experience, or equivalent life experience providing personal care for family or community members, including Elders or individuals with health challenges.

Assets considered for this position are: training in Personal Care Skills, BCLS (Basic Cardiac Life Support) and First Aid Certificates 

This is a Highly Sensitive Position and a satisfactory Criminal Record Check, along with a clear Vulnerable Sector Check is required.

The Official Languages of Nunavut are Inuktut, English and French. Applicants may submit their resume in any of the official languages of Nunavut. Fluency in Inuktitut is required for the position.  Knowledge of Inuit communities, culture,land, Inuit Qaujimajatuqangit, Inuktut and experience working in a northern cross-cultural environment are also considered assets.
