Job Description - Housing Liaison Worker - 6280H

One full-time contract Housing Liaison Worker position available, starting on March 09, 2026, and ending on March 09, 2027, working 37.5 hours per week.


*A vehicle with class G license and 2 million liability is required

Your Role:



  • Establishes and maintains case files.

  • Ensures the completion of all documentation to assist with accurate statistical collection.

  • Completes program-reporting requirements and documentation.

  • Assists in the compilation of information to assist with research and development.

  • Works with community partners to develop a viable sustainability plan for the program.

  • Develops relationships with private market landlords and property management groups to secure apartments/rental units for individuals experiencing chronic homelessness.

  • Liaises with the PATHS2Home team to successfully match landlords with participants needing housing units.

  • Collaborates with the PATHS2Home team to develop a standardized process to house participants.

  • Works with PATHS2Home team to identify participant complex housing-related needs and assists them to develop and execute strategies to promote housing stability.

  • Provides support to landlords/property managers to enable them to sustain relationships with participants identifying with complex needs.

  • Engages in creative problem-solving to address landlord needs and concerns. Maintains a high level of diplomacy in communicating with landlords and community partners.

  • Visits and checks properties in relation to participants’ housing needs. Works collaboratively with the PATHS2Home team and community partners to ensure a coordinated approach to services.

  • Works in alignment with the Agency’s commitment to equity to further create a sense of belonging at the Agency.

  • The above tasks are representative but not all inclusive.


Your Talents and Experience:



  • A degree or diploma in social services with relevant experience working in a community-based setting.

  • Experience in supportive counseling and case management.

  • Knowledge of the local housing market and community services within the K-W community.

  • Excellent oral and written communication skills.

  • Reliable transportation is essential for this position.

  • Required to hold a “G” class license and a two-million-dollar liability policy as part of personal car insurance.
