Shared Health leads the planning and coordinates the integration of patient- centered clinical and preventive health services across Manitoba. The organization also delivers some province-wide health services and supports centralized administrative and business functions for Manitoba health organizations.
The Indigenous planning Coordinator supports a safe coordinated and culturally appropriate discharge planning for Indigenous patients and families within Shared Health. The role will be in collaboration with patients’ families, care teams, Indigenous Resource workers and community partners to support smooth transition to urban, rural, and northers situated communities, while encompassing trauma informed and culturally safe and that is patient-centered.
The position is responsible for responding to and triaging inquiries from patients, visitors, staff, and stakeholders; maintaining accurate records and databases; and assisting with reporting, correspondence, and presentation materials. The Administrative Assistant plays a key role in supporting referral processes, facilitating communication with external agencies, and ensuring timely information flow to program leadership.
This position contributes to building a culturally safe, respectful and trauma-informed workplace, while upholding Indigenous ways of knowing, being and doing, as well as recognizing the impacts of intergenerational trauma and inequities in access to health for Indigenous peoples.
