Location: Health Sciences North, Sudbury Ontario


Full-Time:  18 Months – Maternity Leave (with possibility of extension)


Salary Range:  $72,570 - $88,581


 


The opportunity. Maamwesying North Shore Community Health Services provides primary health, traditional healing, mental health, home and community support services and health promotion services to community members of the seven First Nations of the Mamaweswen North Shore Tribal Council, is inviting applications for the Indigenous System Navigator to join the Home and Community Support Services Team. Under the direction of the Director Home and Community Support Services, the Indigenous System Navigator is responsible for providing advocacy, care coordination and hospital discharge planning supports for individuals who reside within the member First Nation communities of the North Shore Tribal Council, transition from hospital to home.


The Indigenous System Navigator, will assess and determine patient care needs and eligibility, provide access and referrals to community services, and engage with patients, caregivers and other health care practitioners to ensure a smooth transition of care.


The Indigenous System Navigator plays a lead role in providing connected, accessible, patient-centred care – and be supported by our collaborative team that includes regulated health care, other professionals and community services.

What you need to bring with you.


·      Membership, in good standing, with the applicable regulatory body:


•           College of Nurses of Ontario


•           Ontario College of Social Workers and Social Service Workers


·      Recent experience in community health or a related field


·      Knowledge of the health care delivery system and community support resources


·      Cultural competency with knowledge of the health issues affecting Indigenous people



  • Experience working with diverse patient groups, e.g., multicultural, homeless, palliative, acquired brain injury (ABI), mental health, geriatrics, paediatrics

  • Excellent interpersonal, communication, assessment problem-solving, and decision-making skills

  • Effective time management, prioritization and organizational skills, with the ability to work independently and co-operatively in a busy multidisciplinary environment

  • Established ability to accurately complete required documentation, reports and forms

  • A valid driver’s licence and access to a reliable vehicle

  • Proficient in a Windows environment

  • A clear vulnerable sector criminal reference check is a condition of employment

  • Ability to speak Ojibwe is a definite asset

We have recently been approved as the first Indigenous lead Ontario Health Team by the Ministry of Health.


 


What to expect. Given the traditional practices of Indigenous people, from time to time exposure to smoke from the burning of sacred medicines such as tobacco, sweet grass, sage, or cedar may occur.
