HOURS OF WORK: 35 Hours per week 


UNIONIZED POSITION: OPSEU Local #332


COMPENSATION: $35.43 to $37.60 per hour

INTRODUCTION: 


New Path Youth and Family Services is an accredited children's mental health agency providing a range of compassionate, innovative and accessible services for children, youth and their families throughout Simcoe County. New Path respectfully acknowledges we are situated on the traditional land of the Anishinaabeg people which includes the Odawa, Ojibwe, and Pottawatomi Nations collectively known as the Three Fires Confederacy. 



PURPOSE OF THE POSITION:


The Intensive Services and Transition Worker provides evidence based/informed programs to youth and their families in their home, school and community to assist with transition planning and support gains made through Live-In Treatment Service (LITS) or community services/programs. 



JOB DUTIES AND RESPONSIBILITES:



  1. Meet with children, youth and/or families at home, school, in-office and/or in the community to further support treatment gains and transition plans utilizing evidence based/informed programs.

  2. Participate in case planning meetings to provide input into the treatment goals and objectives, plans for service, and programs achieving the goals of clients.

  3. Develop collaborative client transition plans and implement these plans across all life domains, including out-of-region supports.

  4. Deliver evidence based/informed intervention services through individual, family or group meetings.

  5. Facilitate groups for children, youth and parents/guardians.

  6. Work in partnership with the Hospital’s Child and Youth Mental Program to provide transition support to youth and their families.

  7. Work as a part of a multidisciplinary treatment team in delivering client services and act as case manager for assigned clients when required.

  8. Perform Crisis Intervention, Medication Administration and First Aid/CPR.

  9. Complete centralized intake screenings and presentations.

  10. Provide support to LITS program as required.

QUALIFICATIONS: 



  1. Child and Youth Worker Diploma.

  2. Minimum 2 years’ experience working with children, youth and their families in a children’s mental health or related service.

  3. Experience working with evidence based/informed programs.

  4. Knowledge of youth and family engagement strategies and behavioural interventions for working with children, youth and families.

  7. Demonstrates awareness for the backgrounds, values, beliefs and traditions of staff, clients, families and communities we serve. 

  8. Continuously assesses and challenges assumptions, personal unconscious bias and commits to understanding diverse perspectives, experiences and their intersectionality. 

  9. Commits to ongoing training to build the knowledge, awareness and skills, that contribute to creating a culturally safe environment for staff, clients and families. 

  10. Competency and effective use of database and Microsoft Office systems. 

  11. Confident, articulate and professional when communicating verbally and in writing. 

  12. French Language is an asset. 



WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

  1. Ability to work flexible hours including some evenings, weekends, and holidays.

  2. This position is primarily performed in the community/ office/on-site to support the in-person meeting preferences of our clients and families. Some administrative tasks (non-client facing work) can be requested to be completed remotely (Work From Home).

  3. Must be able to effectively deliver crisis intervention using a Ministry approved training program including physical techniques. Training will be provided.

  4. Travel is required to other agency offices, community locations, and in-home visits within Simcoe County and York Region using a personal vehicle. A valid driver’s license with a minimum G2 is required.

  5. Prolonged periods of sitting and working on a computer.
