Gattuso Centre for Social Medicine
The Gattuso Centre for Social Medicine seeks to address health inequities, working alongside community organizations, UHN clinicians, marginalized populations, persons with lived experience, patients, and all levels of policymaking to co-design, evaluate, and scale innovative data-driven programs that lead to better health. Applying a social determinants of health approach, the Social Medicine and Population Health Program aims to bridge health services and the broader social environment of UHN’s most medically and socially complex patients with the highest needs through the design, implementation, research and evaluation of new care models and initiatives to address the health implications of inequity.
Position Summary
As a member of Gattuso Centre for Social Medicine Team, the Program Operations Lead works in support of and in good collaboration with the Social Medicine leadership team, Senior Manager, Community Health Worker Lead, clinical team and community partners to provide leadership and direction for program operations and primary care services. The position will support front line, clinical activities as part of the Social Medicine services, including the implementation and evaluation of new models of care, day to day management of the services, and is a unique opportunity in the field of social medicine at a first-of-its-kind Centre.
The principal responsibilities include: maintaining responsibility for the effective and efficient management of the clinical services; providing leadership and direction to the interprofessional and administrative team and community staffing partners operating in Social Medicine program spaces; assuming responsibility for the effective oversight of operational details, patient care and quality; performing corporate management responsibilities; performing cross-functional duties, as assigned or requested; performing other duties consistent to the job classification, as required.
