Gattuso Centre for Social Medicine
The Gattuso Centre for Social Medicine seeks to address health inequities, working alongside community organizations, UHN clinicians, marginalized populations, persons with lived experience, patients, and all levels of policymaking to co-design, evaluate, and scale innovative data-driven programs that lead to better health. Applying a social determinants of health approach, the Social Medicine and Population Health Program aims to bridge health services and the broader social environment of UHN’s most medically and socially complex patients with the highest needs through the design, implementation, research and evaluation of new care models and initiatives to address the health implications of inequity.

Position Summary

As a member of Gattuso Centre for Social Medicine Team, the Program Operations Lead works in support of and in good collaboration with the Social Medicine leadership team, Senior Manager, Community Health Worker Lead, clinical team and community partners to provide leadership and direction for program operations and primary care services. The position will support front line, clinical activities as part of the Social Medicine services, including the implementation and evaluation of new models of care, day to day management of the services, and is a unique opportunity in the field of social medicine at a first-of-its-kind Centre.

The principal responsibilities include: maintaining responsibility for the effective and efficient management of the clinical services; providing leadership and direction to the interprofessional and administrative team and community staffing partners operating in Social Medicine program spaces; assuming responsibility for the effective oversight of operational details, patient care and quality; performing corporate management responsibilities; performing cross-functional duties, as assigned or requested; performing other duties consistent to the job classification, as required.

Duties

  • Responsible for the day to day management and organizational effectiveness of the clinical operations in Social Medicine program spaces
  • Ensures program goals and objectives are aligned with the hospital’s strategic business objectives
  • Facilitates the interprofessional team model, ensuring that the model of care delivery is consistent with UHN mission, vision, values and strategic plans
  • Facilitates the development of service delivery solutions by collaborating and/or coordinating with community partners, physicians, managers of other interprofessional teams, units/departments to ensure continuity and optimal patient care and social support
  • Supports the implementation of best practices, quality documentation and accountability models
  • Evaluates the patient care and social support, planning and implementation processes within the department; liaising and collaborating with the Management Team to ensure the program’s patient population is provided optimal care
  • Communicates and disseminates relevant information to staff through team meetings, in-services and other appropriate communication media (e.g. email, shared drive)

Qualifications

  • Successful completion of a bachelors’ degree in a regulated health profession or recognized equivalent required
  • At minimum four years practical and related experience, experience in a hospital or primary care environment would be an asset.
  • Strong knowledge of, and familiarity with the principals of population health and health equity
  • Experience in maintaining high quality practice in the clinical and community environment
  • Ability to provide duties in a professional manner and produce high quality work
  • Exercise and initiative and good judgement with the ability to multi-task
  • Compliance with confidentiality requirements
  • Effective organizational, communication, and interpersonal skills
  • Knowledge of applicable legislative, UHN and/or departmental policies
  • Client service oriented, with the ability to effectively work with diversity and appreciate that people with different opinion, backgrounds and characteristics bring richness to the challenge or situation at hand
  • Leadership or project management experience preferred
  • Membership in good standing with a Regulated Health Professional College, if applicable
