Job Description - Manager, Hospice Palliative Care

POSITION SUMMARY   



Better Living Health and Community Services has a permanent full-time opening for the position of Manager, Hospice Palliative Care will be responsible for the management of health, wellness and continuity of service delivery for clients and residents and their families; supervision of Hospice Program staff and advancing Palliative Care in the Better Living catchment area.  This position will be split between Better Living Hospice program locations in Newmarket and Toronto. 





This position reports to the Vice President Hospice Palliative Care


 


REQUIRED QUALIFICATIONS


 



  • Undergraduate degree or post graduate education in a related health or social services discipline

  • Minimum of 5 years experience working in health or human services, or equivalent experience

  • Minimum of 3 years management experience leading a team in the delivery of health or human services

  • Working knowledge of psychosocial assessments, and working with electronic records and in a client data base system

  • Must possess a valid Ontario Driver’s License and access to a vehicle for the purposes of business travel



SKILLS and KNOWLEDGE


 



  • An understanding of the history, philosophy, and services of hospice palliative care and the model of care provided by Better Living Health and Community Services

  • Management experience with staff supervision, program development, growth, evaluation, and budgeting

  • Ability to establish and maintain a positive and collaborative work environment focused on empowering employees to achieve organizational objectives

  • Understanding the importance of community engagement and partnership building for the advancement and delivery of exceptional programs and services

  • Experience developing professional and effective working relationships with staff, students, volunteers, peers, stakeholders, community members and funders

  • A demonstrated commitment to continuous learning and improvement. The successful candidate will have a track record of seeking out new ideas, pursuing and achieving goals and using lessons learned to improve the performance and effectiveness of the hospice palliative care programs

  • Effective listening skills including the ability to seek clarification as necessary and an openness to understanding and integrating the ideas of others.

  • Experience working with and meeting accreditation or quality standards



KEY OPERATIONAL RESPONSIBILITIES




  • Oversee the operations of the psychosocial team including case management, visiting hospice services, caregiver support, grief and bereavement and art therapy

  • Manage the day to day Human Resources within the department (including program students and volunteers), ensuring effective teamwork, high standards of work quality and organizational performance

  • Responsible for the growth, development, implementation and evaluation of results-based programs that improve and enhance client-centered service delivery and operational performance

  • Provide client and caregiver support in the absence of psychosocial staff at all hospice programs

  • Complete monthly statistical reporting, monthly and quarterly reports

  • Manage the Community Hospice budgets

  • Provide client and caregiver support in the absence of psychosocial staff

  • Participate in the rotational on-call system with Director of Care at Margaret Bahen Hospice

  • Support PalCare, and the NYTHP and NYSS OHT’s with education to health care providers and community members as requested

  • Pursue continuous learning and improvement by seeking out new ideas, identifying and achieving goals and using lessons learned to improve the performance and effectiveness of the hospice palliative care programs

  • Actively participate in the development of policies and procedures in compliance with accreditation standards, quality initiatives and funder requirements

  • Lead program promotion and community outreach activities

  • Support the writing of grants as applicable to hospice programs

  • Maintain compliance with the policies and procedures of the organizations.

  • Adhere to the Personal Health Information Protection Act (PHIPA 2010) as outlined in the organization privacy policies and procedures.

  • Perform the functions of job description in compliance with the Health and Safety practices of the organization and in accordance with the provincial Occupational Health and Safety Act. 

  • Participate in the continuous quality improvement activities of the organization in order to advance a culture of quality and support ongoing compliance with CARF and HPCO Accreditation standards.

SALARY: $73,920 to $77, 000 per annum
