Job Description - NSWOCC-Certified Wound Care Nurse Needed in GTA Area for Community-Based Clients

The Care Company is seeking a compassionate and skilled NSWOCC Wound Care Nurse to join our dedicated healthcare team. This role is ideal for a nurse who is passionate about delivering high-quality, client-centred care in the community while working with a boutique organization that values collaboration, flexibility, and a personalized approach to nursing.

**We are only considering applicants who are Registered Nurses with the CNO and have an NSWOCC certification**

Responsibilities

  • Deliver advanced clinical expertise in the assessment, planning, implementation, and evaluation of care for clients with wound, ostomy, and continence needs.
  • Collaborate with families and the interdisciplinary care team to develop and maintain individualized care plans.
  • Educate and support clients and caregivers on wound, ostomy, and continence management.
  • Serve as a clinical resource and mentor for nursing colleagues within The Care Company.
  • Maintain accurate documentation in compliance with professional and organizational standards.
  • Contribute to quality improvement initiatives and best practices in client care.

  • Registered and in good standing with the College of Nurses of Ontario (CNO).
  • **Wound, Ostomy and Continence Education Program (WOC-EP) from NSWOCC**
  • Preference will be given to candidates who have completed any of the following: (1) a master’s program in Advanced Wound Care (MScCH WPC) from UofT, or (2) a Master of Clinical Science (MCISc) in Wound Healing Program from Western University or (3) an education program with the International Interdisciplinary Wound Care Course (IIWCC) from UofT.
  • RN diploma/degree from a recognized university or college.
  • A moderate level of wound care or venipuncture knowledge and experience is an asset.
  • Knowledge of community nursing is an asset.
  • Demonstrate effective communication (written and verbal) and documentation skills.
  • Demonstrate reliable attendance, punctuality, and availability for the role.
  • A valid Ontario driver’s license (G License) and access to a reliable vehicle is an asset.
  • Current CPR and First Aid certification, completed through a recognized training provider within the past 12 months.
  • Valid WHMIS 2015 certification, issued or renewed within the past 12 months.
  • Negative TB test (2-step or 1-step with prior documentation), completed within the past 12 months.
  • Proof of full COVID-19 vaccination, or exemption in accordance with the Ontario Human Rights Code.
  • Clear Criminal Record Check with Vulnerable Sector Screening, issued within the past 12 months.
  • Must be able to communicate effectively in English, both orally and in writing, for documentation and communication with clients and care teams.
  • Must be comfortable and capable of providing care to both male and female clients of varying ages and care needs.
