Job Description:


Within the context of a client and family centered model and in accordance with the SRBA standards of professional practice, code of ethics and the established vision and values of the health authority, the Palliative care coordinator provides assessments and access to a comprehensive range of palliative care services. The Palliative care Coordinator, in consultation with other team members will have primary responsibility for determining eligibility of clients for palliative care services home care program, and its components based on established criteria. The Coordinator provides professional advice to clients with palliative care needs; facilitates regular discussions with clinicians in different settings to determine the most appropriate level of care for clients during different phases of their illness and facilitates access for clients to all components of palliative home care acute palliative care beds, hospice and long term care.


 


Licenses


  • Canadian Hospice Palliative Care Nurse Certificate (CHPCN)



 


Education


  • Bachelor of Science in Nursing (BScN)

Experience


  • Minimum of three (3) years relevant long term care nursing experience

  • Experience in program integration and inter-professional practice desired

  • Knowledge of professional nursing theory and practice

  • Knowledge of Occupational Health and Safety Standards and application of such

  • Knowledge of palliative care and end of life care strategies

  • Ability to exercise independent nursing judgement

  • Ability to use compassionate sensitive approach relating to victims & families

  • Ability to manage multiple tasks at various stages according to priorities.

  • Knowledge of causes & effects of musculoskeletal injuries & chronic pain mgmt.  

  • Enhanced knowledge of clinical assessment

  • Ability to build and maintain effective relationships/partnerships

  • Knowledge in family centered care

  • Demonstrates the ability to be resourceful and to adapt to changing conditions

  • Ability to work in a high stress, fast paced work environment with complex cases

  • Knowledge of palliative care and end of life care strategies

  • Demonstrated ability to assist with informed decision making in the midst of suffering



 


Other Information


  • Travel required to various community sites within Regina PHC Networks

  • Willing to Travel to rural areas as assigned. Travel allowance provided.

  • Experience in palliative care considered an asset.

  • Providing clinical experience including assessment and care planning for clients

  • Community based position provides services in clinic/home settings in PHC Regina

  • Must possess a valid driver's license and have a vehicle available for work.

  • Promoting excellent patient care through expert consultation and role modeling

  • Registered/eligible with the College of Registered Nurses of Saskatchewan (CRNS)

  • Case managing clients on the palliative home care program and making referrals as needed for clients
