Reporting to:                   Director, Integrated Health Services


Compensation:                $61.97 - $71.66/hr plus a comprehensive compensation package that includes time-off entitlements, health benefits, life insurance, and participation in the HOOPP pension plan

What You’ll Do



Reporting to the Director of Integrated Health Services, the Nurse Practitioner (NP) is a key member of the interdisciplinary team, delivering comprehensive primary health care services within the Primary Health Care (PHC) clinic. The NP provides care to a rostered group of clients within the practice, working to their full scope of practice. They deliver culturally responsive care both independently and in collaboration with an interprofessional team. The successful candidate will demonstrate the ability to work autonomously and as part of a team, with a strong commitment to culturally safe, equitable, and client-centered care. This position requires availability to provide service one evening per week, until 8:00 p.m.


 


The NP is responsible for delivering primary health care services in accordance with the Standards of Practice for Registered Nurses in the Extended Class and the Expanded Nursing Services for Patients Act. This includes health promotion, counseling, disease prevention, diagnosis, treatment, and referrals to specialists or diagnostic testing. Providing clinical services, requiring the NP to manage a diverse range of health conditions while addressing the complex needs of the populations served.


 


The NP plays an active role in the development, implementation, monitoring, and evaluation of programs and services, ensuring accessible and high-quality care for individuals with diverse health, social, cultural, and economic needs. Functioning both autonomously and within a collaborative team environment, the NP contributes to the advancement of program goals, enhancing health outcomes within the community.

What We’re Looking For 



Your Education Path



  • Current extended class certificate (NP-PHC) and unrestricted registration with the College of Nurses of Ontario 



  • Knowledge of ODSP application process

  • Certification in CPR (BLS-C)



Professional Experience



  • Minimum two years’ experience as a Nurse Practitioner working in a primary care setting

  • Strong clinical skills and diagnostic abilities for clients across the life span

  • Previous experience working with culturally diverse and marginalized populations

  • Interest in providing outreach services


 


Language Capability



  • English is required

  • Other languages are an asset.



What You Bring to the Table




  • Knowledge of primary health care and the social determinants of health 

  • High sensitivity to social, spiritual, emotional, and physical health needs of ACB communities



  • Knowledge of concepts of health promotion, disease prevention, behavior change counselling, program planning, individual and group counselling 

  • Knowledge and awareness related to cultural competence

  • In-depth knowledge of health assessment and interventions in the family practice context 

  • Knowledge of non-violent crisis intervention 

  • Ability to work independently and be self-directed 

  • Strong understanding of professional practice obligations and seek consult with regulatory college as required to support sound and ethical clinical practice.

  • Excellent communication skills and ability to adapt communication styles to meet the needs of clients, health team members and community partners

  • Ability to work under pressure both independently and as part of a team

  • Successful completion of Police record Check for vulnerable populations
