Job Description - Nurse Practitioner (Extended Class) - COHT Care Clinic

Position Summary

The Nurse Practitioner (NP) Extended Class (EC) for the Couchiching OHT (COHT) Care Clinic uses advanced practice clinical skills to provide direct clinical care, education, research, consultation and treatment to support the care needs of patients at the COHT. The NP will establish relationships with physicians, nurses, discharge planners and other COHT partners and OSMH staff to enhance the model of integrated care.

PRIMARY RESPONSIBILITIES:

•Works actively with the COHT Care Clinic team throughout the shift to prioritize patient care needs
•Visit patients in their homes for assessment, treatment, and advanced clinical management as needed
•Be the Most Responsible Practitioner (MRP) for patients whose care needs can be met within the NP scope of practice at the COHT Care Clinic and Community Partner Organizations as needed
•Consult as needed with the Hospitalists, Specialists, Community Primary Care Practitioners and Allied Health team members
•Within a model of collaborative practice, the NP uses advanced theoretical knowledge base and clinical competence and skill to assess, plan, provide and facilitate care across a continuum of care
•Assess patients/families through multiple strategies, including comprehensive admission history and interview, advanced physical examination, interval assessments, and the interpretation of diagnostic testing to determine need for medical, nursing, or other care
•Utilizes diagnostic reasoning and decision-making process to formulate a list of hypotheses, determine the need for, order, and interpret diagnostic testing, and formulation of a medical diagnosis within the specialty scope of practice and consistent with practice guidelines
•Communicates significant clinical findings to consulting physicians and facilitates ongoing communication of patient progress and plan of care to the health care team
•Initiates/orders, tailors and evaluates/discontinues selected medical and nursing therapy (including pharmacologic therapy) within the specialty scope of practice and consistent with practice guidelines and established medical directives if needed for NP to optimize the plan of care
•Performs specialty-specific diagnostic and therapeutic interventions in accordance with scope of practice, approved medical directives for controlled acts and established practice guidelines
•Facilitates patient care by consulting with the patient/family and inter-professional patient care team to formulate, initiate, co-ordinate and evaluate a comprehensive and collaborative patient centred plan of care, ensuring effective and efficient use of resources
•Promotes continuity of care within the Couchiching Ontario Health Team by initiating and co-ordinating patient discharge plans, warm transfers and integrated care methodology
•Writes/dictates consultations, progress notes, and discharge summary reports
•Evaluates patient care outcomes and nursing practice issues to ensure the highest possible quality of care.
•Supervises/Precepts nursing students (graduate and undergraduate) and other interdisciplinary students as required in their clinical experience and evaluates their performance
•Provides clinical nursing leadership to promote professional development to assist nursing staff in the application of knowledge/theory to practice through role-modelling and coaching, and by participating in formal staff education programs in collaboration with the program leadership team
•Provides leadership in the critical analysis and review of literature and the application of research findings to practice through the development and implementation of clinical practice guidelines and standards of care
•Identifies and implements strategies to translate and interpret research findings and theoretical frameworks into practice
•Facilitates the utilization of research by nursing and inter-professional staff and contributing to knowledge base through presentation or publication
•Support program planning, implementation and evaluation as well as project management
•Safety and risk management
•Participating in a supportive and healthy working environment
•Performs other duties as may be assigned by program leadership
•It is a requirement that the NP will works a variety of shift types including days, evenings, weekends and holidays

Qualifications

Education:
•Current registration with the College of Nurses of Ontario is required
•BScN required
•Nurse Practitioner Certification required
•Masters degree in Nursing/Nurse Practitioner Stream is preferred
•Primary Care Nurse Practitioner Certification preferred
•BCLS is required

Experience:
•Minimum three (3) years’ experience in advanced practice nursing in primary care required
•Minimum three (3) years relevant clinical specialty-related experience as a Registered Nurse required, experience in Primary Care preferred
•Recent experience in a leadership role within an inter-professional team preferred
•Relevant knowledge of integrated care environment preferred

•Demonstrated program development skills and experience
•Ability to work with minimal direction to successfully achieve objectives
•Knowledge and proficiency in current evidence-based methods and practices of primary care delivery, with an emphasis on health promotion and risk reduction
•Demonstrated commitment to the concepts of Continuous Quality Improvement and Customer Service

•Clinical specialty organizational membership
•Current member of the Nurse Practitioner’s Association of Ontario required
•Demonstrates commitment to continued professional and personal growth and development through Continuing Education Programs and Self-Directed Learning Opportunities.
•Must have a valid driver’s license and a reliable vehicle to provide client care / services off-site.
•Computer literacy working in Windows environment
