Job Description - Nurse Practitioner (Extended Class) Renal Program

Position Summary

POSITION SUMMARY:
The Nurse Practitioner (NP) (Extended Class) for the Regional Kidney Care Program (RKCP) uses advanced clinical practice skills to provide direct clinical care, education, research, consultation and treatment to support the care needs of designated renal patients. The NP in the renal program will function as an independent practitioner and as a collaborative partner within the inter-professional team.

The Renal NP has an interest in chronic kidney care management and function in an expanded nursing role with a high level of autonomy. Working in collaboration with Physicians, and Nephrologists, the Renal NP will use expert skill to formulate clinical decisions and appropriately manage the care needs of both our in-patient and out-patient Regional Renal patients.

As an advanced practice nurse, the NP will provide leadership and support to the professional practice and growth of the team through mentoring, collaboration, and education. The Renal NP is part of the leadership team of the Regional Renal Program Providing leadership, knowledge, development, of standards/processes, and quality initiatives enhancing the care for our Renal Patients.


PRIMARY RESPONSIBILITIES:
•Performing and documenting medical history and physical exam finding in the Electronic Medical Record (EMR)
•Diagnosing medical conditions
•Ordering appropriate tests/procedures in consultation with physician partners
•Interpreting laboratory/diagnostic imaging and other test results
•Prescribing pharmacologic and non-pharmacologic therapies independently or in consultation with physician partners
•Consult as needed with the Hospitalists, Specialists, Community Primary Care Practitioners and Allied Health team members
•Utilizes diagnostic reasoning and decision making process to formulate a list of hypotheses, determine the need for, order, and interpret diagnostic testing, and formulation of a medical diagnosis within the specialty scope of practice and consistent with practice guidelines
•Initiates/orders, tailors and evaluates/discontinues selected medical and nursing therapy
•Performs specialty-specific diagnostic and therapeutic interventions in accordance with scope of practice, approved medical directives for controlled acts and established practice guidelines
•Supervises/Precepts nursing students (graduate and undergraduate) and other interdisciplinary students as required in their clinical experience and evaluates their performance

Performs other duties as may be assigned by the direct manager.

Qualifications

QUALIFICATIONS:

Education:
•Current registration with the College of Nurses of Ontario is required
•BScN required
•Nurse Practitioner Certification required
•Masters degree in Nursing/Nurse Practitioner Stream is preferred
•Acute Care Nurse Practitioner Certification preferred
•Gerontology Nursing Certification is preferred
•BCLS is required

Experience:
•Minimum three (3) years experience in advanced practice nursing in adult medicine/geriatrics/eldercare preferred
•Minimum three (3) years relevant clinical specialty-related experience as a Registered Nurse required, experience in Acute Care preferred
•Recent experience in a leadership role within an inter-professional team preferred
•Relevant knowledge of inpatient care environment preferred

•Demonstrated commitment to the concepts of Continuous Quality Improvement and Customer Service


Other:
•Current registration with the College of Nurses of Ontario is required
•Current member of RNAO
•Clinical specialty organizational membership
•Current member of the Nurse Practitioner’s Association of Ontario
•Demonstrates commitment to continued professional and personal growth and development through Continuing Education Programs and Self Directed Learning Opportunities.
•Must have a valid driver’s license and a reliable vehicle to provide client care / services off-site.
•Computer literacy working in Windows environment
