The Nurse Practitioner is a Registered Nurse with additional educational preparation and experience who possesses and demonstrates the competencies to autonomously diagnose, order and interpret diagnostic tests, prescribe pharmaceuticals, and perform specific procedures within his/her legislated scope of practice. The NP role is derived from blending clinical diagnostic and therapeutic knowledge, skills and abilities within a nursing framework that emphasizes holism, health promotion and partnership with individuals and families, as well as communities. Nurse practitioners have the knowledge to assess population health trends and patterns and to design services that promote healthy living. They provide leadership in the development, implementation and evaluation of strategies to promote health and prevent illness and injury, and they work with interprofessional teams, other health-care providers and sectors and community members. Nurse practitioners collaborate in the development of policy to influence health services and healthy public policy.
The Nurse Practitioner utilizes a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for medical patients in an inpatient setting in collaboration with Family Practice physicians and the interdisciplinary team. The NP will be expected to provide a comprehensive health assessment and diagnosis of medical problems, develop and implement a plan of care, provide health counseling and emotional support in the areas of illness and injury prevention, complications, and health promotion. Furthermore, the NP will assist the interdisciplinary teams in implementing a plan of care that will enable an overall reduction of length of stay in inpatient units, as well as engage in strategies to prevent or reduce admission/readmission to Hospital.
Additional Responsibilities may include:
Direct Comprehensive Care: Obtains and documents a comprehensive health history, performs a physical examination on patients requiring assessment, collaborates closely with the Internal Medicine physicians and other members of the interdisciplinary team and coordinates care involving other healthcare providers such as the CCAC. Furthermore, the NP has a role for advancing a patient’s plan of care and working with the interdisciplinary team to improve discharge processes and length of stay.
Leadership: Provides leadership in the development, implementation and monitoring and evaluation of quality management programs. Participates and provides leadership in the strategic planning process for the delivery of medical care. Supports educational initiatives directed at nursing and allied health to promote and provide medical care that meets best practice standards.
Education: Contributes to the development, implementation and evaluation of patient education programs. Conducts educational teaching sessions with patients and/or families with a focus on health promotion, disease prevention, and impact of illness experience on the patient and family.
Research: Encourages, supports, and facilitates the development of researchable questions in clinical practice.
Consultation: Provides consultation internally and externally for clinical practice issues within the NP’s area of specialty and scope of practice.
