McMaster Children's Hospital (MCH) is part of the Hamilton Health Sciences' family of hospitals. The MCH Hemoglobinopathy outpatient program provides coordinated, comprehensive care to patients from infancy through adulthood with an interdisciplinary model of care. The Hemoglobinopathy Program provides an inter-professional approach with expertise in pediatric and adult disease management within a family-centered environment.

Position Summary
The Nurse Practitioner (NP) for the Hematology service of McMaster Children’s Hospital will provide leadership and promote excellence in comprehensive care across the life spectrum of patients with inherited blood disorders - particularly Sickle Cell Disease and Thalassemia. Regarding providing comprehensive care, the NP will demonstrate excellent understanding of the inequities regularly encountered by populations experiencing inherited blood disorders – particularly Sickle Cell Disease – and will demonstrate experience working with these populations to mitigate these inequities and improve patient experiences and outcomes. The Nurse Practitioner is a core member of the interdisciplinary Hemoglobinopathy Clinic team. They will engage in family-centered and holistic nursing care through expert clinical practice, education, consultation and research. The NP will support the clinical activity of the Hematology service and the associated patient care and interventions with a focus on care for Hemoglobinopathy patients. The NP will collaborate with the interdisciplinary team and will be responsible to: provide direct patient care, coordination of care including transfusion therapy, discharge planning, education for patient, family and their communities, and the coordination of related community and other resources for this patient population. The NP will also be responsible to mentor nursing and other health professional students as appropriate, and participate in continuing education activities to integrate the role of the NP in the program and academic settings. This includes initiating, participating, and/or leading programs, presentations, publications, evaluations, research initiatives and other system changes to continue the enhancement of interdisciplinary care teams.
