Trillium Health Partners (THP) is one of Canada’s largest community-based teaching hospitals, serving the growing and diverse populations of Mississauga, West Toronto, and surrounding communities through the Credit Valley Hospital, the Mississauga Hospital, the Queensway Health Centre, the Reactivation Care Centre (Church Site) and the THP-UHN Reactivation Care Centre. Guided by our values of compassion, excellence, and courage and through our strategic roadmap, Plan to 2030, we are creating a new kind of health care - defined not by illness, but by the health and well-being of people and communities.
Transforming healthcare through innovation, Insight Health Solutions, fully part of THP, is a provider of private pay and third-party funded services including assessment, rehabilitation and disability management services (e.g. WSIB, insurer, employer). Insight Health Solutions has grown significantly over 25 years to be a leading provider of hospital-based third-party clinical services across Ontario to a broad spectrum of customers and clients. Insight Health Solutions works closely with other innovative, entrepreneurial and evidence-informed groups to exceed our customer and client needs. One of our latest projects is the launch of Infusion Clinic – a first for Insight Health Solutions!
Position Summary:
Reporting to the Program Manager at Insight Health Solutions, the Nurse Practitioner will utilize their comprehensive theoretical knowledge base and advanced level of clinical competence and skills to support the development and clinical needs of exciting innovative programs at Insight Health Solutions. This role is intended to provide flexible clinical coverage across Solutions Clinic and other emerging initiatives.
This role will primarily include supporting the delivery of Infusion Clinic services aimed at enhancing the efficient delivery of intravenous iron (IVFe). The NP will also provide in-person medical assessment and treatment services to employers, and patients directly. Furthermore, the Nurse Practitioner will support IHS' Occupational Health services for employers, businesses and other clients, including pre-employment occupational health assessments, fitness-to-duty examinations, point-of-care testing (POCT), laboratory specimen collection, immunizations, general physical examinations, and other occupational health medical assessments, as required.
This role also offers the opportunity to support THP Solutions Clinic, an innovation initiative delivering virtual care to the community. The program currently delivers physician to physician support through the Seamless Care Optimizing Patient Experience (SCOPE) program; medical assessment and treatment in collaboration with the Community Paramedicine Program, virtual specialist care to nursing homes, and to patients directly. The program has launched a pilot clinic in October 2025 to further scale and test the virtual care platform to offer patients with lower acuity issues to access an assessment with an NP in a HealthPod. These self-contained private and secure HealthPods are equipped with biometric devices and virtual care-enabling technologies that connect patients with healthcare providers for high-quality, remote medical assessment and treatment. Coverage to be provided in this practice area should this be required based on operational need.
The Nurse Practitioner (NP) will utilize a comprehensive theoretical knowledge base and an advanced level of clinical competence in caring for patients within these programs. This will involve performing advanced patient care assessments and coordinating care for patients with complex co-morbidities, ordering appropriate diagnostic tests, pain and symptom management, and patient education. The NP will be instrumental in helping patients navigate the system to improve access and optimize safe and high-quality outcomes thereby, enhancing a positive patient experience closer to home.
