Nurse Practitioner (NP)

Better Living at Thompson House is seeking a compassionate and experienced Nurse Practitioner (NP) to provide comprehensive primary care services to residents in our long-term care home.

The Nurse Practitioner reports to the Director of Care, with accountability to the Medical Director, and collaborates with residents, families, and the interdisciplinary health care team in the development, implementation, and evaluation of individualized plans of care.

The NP also provides leadership and mentorship to nurse managers and clinical staff to enhance clinical knowledge, assessment skills, and quality of resident care. This role is pivotal in enhancing residents’ quality of life while reducing unnecessary emergency department visits and hospital transfers through proactive, resident-centered care.
 

The Successful Candidate For This Position Must Have The Following Qualifications:    

• Current registration in good standing as a Nurse Practitioner (Extended Class) with the College of Nurses of Ontario.


• Graduate degree in Nursing with NP certification.


• Experience in long-term care, gerontology, or primary care preferred.


• Strong clinical assessment and diagnostic skills.


• Knowledge of long-term care legislation and standards.


• Experience managing complex and chronic health conditions in older adults preferred.

The Successful Candidate for this Position Must be able to:    

• Provide comprehensive primary care services within the NP scope of practice.


• Perform health assessments, diagnose conditions, order and interpret diagnostic tests, and prescribe treatments and medications as appropriate.


• Develop, implement, and evaluate resident care plans in collaboration with the interdisciplinary team.


• Support timely clinical interventions to prevent avoidable hospital transfers.


• Provide chronic disease management and palliative care support as appropriate.


• Collaborate with attending physicians, specialists, and external health care providers.


• Participate in admissions, re-admissions, and complex care planning.


• Provide clinical leadership and mentorship to nursing and clinical staff.


• Support quality improvement initiatives and evidence-based practice.


• Participate in outbreak management, infection prevention, and clinical risk management initiatives.


• Contribute to policy development, clinical protocols, and program improvements.


• Support family conferences and complex care discussions as required.


• Ensure documentation meets professional and legislative standards.

Working Conditions

Works collaboratively within the Home and with external partners. Participation in on-call or extended coverage may be required based on operational needs.
