Our supports are tailored to each person’s goals, preferences, strengths, and level of support needed. We provide residential and community-based programs, including Residential Living, Supported Independent Living, Home Share, Respite, and Cluster services.

The Role

As an Outreach Support Mentor, you will provide one-on-one support to individuals within our Supported Independent Living, Outreach, and Respite programs, both in their homes and in the community.

During your shift, you may support multiple individuals throughout the day, helping them build independence, develop life skills, access community resources, attend appointments, and participate in meaningful activities.

Although you work one-on-one with the individuals you support, you are not alone. Outreach Support Mentors collaborate closely with other team members to ensure the needs outlined in support plans are met and to empower individuals to achieve their personal goals.

How You'll Make a Difference

• Building positive, professional relationships with individuals, families, and team members


• Supporting individuals to build independence and develop daily living skills


• Connecting individuals with community resources such as food banks, clothing resources, and other supports


• Supporting individuals with medical appointments and community access


• Administering medications according to established procedures


• Supporting grocery shopping, meal preparation, cooking, and household routines


• Supporting individuals with bill payments and money management


• Teaching and supporting transportation skills, including learning how to use public transit


• Supporting individuals to acquire and maintain productive roles such as employment, volunteering, and programming


• Supporting access to educational, recreational, and community programming


• Supporting recreational opportunities such as skating, swimming, sporting events, and other activities


• Providing emotional support and companionship


• Supporting individuals during crisis situations using prevention, intervention, and de-escalation strategies


• Supporting personal care and daily living needs as required


• Maintaining a clean, safe, and welcoming home environment


• Maintaining accurate documentation and reporting to support continuity and quality of support


• Advocating for the rights, choices, preferences, and goals of the individuals supported

What You Need to Be Successful

• Proof of legal eligibility to work in Canada will be required as part of the recruitment and onboarding process


• High School Diploma


• Post-secondary education or courses in Community Support Work, Disability and Community Support, Social Services, Mental Health, or a related field is considered an asset


• 2 years of experience supporting adults with intellectual or developmental disabilities preferred


• Knowledge of disabilities, disorders, and mental health challenges


• Strong verbal and written communication skills


• Advocacy and problem-solving skills


• Ability to work independently and collaboratively


• Flexibility and adaptability


• A valid Class 5 Driver's Licence


• Reliable access to a personal vehicle


• Ability to travel between locations as required

Extended Background Checks Including the Following

• Criminal Record Check with Vulnerable Sector Search


• Adult Abuse Registry Check


• Child Abuse Registry Check


• Current First Aid and CPR Certification

Hours of Work

As we provide 24/7 support, a variety of shifts may become available based on the operational needs of the individuals and programs we support.

Future Outreach Support Mentor opportunities may include shifts between 10:00 a.m. and 8:00 p.m., Monday through Sunday.

Schedules will vary based on program and individual needs. Applicants are encouraged to indicate their availability when applying.

Work Location

South-Eastern Manitoba

Applicants should be comfortable commuting to communities located southeast of Winnipeg. Specific work locations will be discussed with shortlisted candidates during the recruitment process.
