Job Description - (Permanent Full-Time) Case Worker - Central Intake

Description


The Case Worker plays a key role in the Central Intake department by supporting client-centered intake, screening, navigation, and service coordination across the mental health and addictions (MHA) system. This role facilitates smooth access to care by reviewing referrals, engaging clients and families, conducting brief screening, and linking clients to appropriate services.


Key Responsibilities


 Client Intake, Screening & Navigation



  • Review incoming referrals and determine eligibility for Ontario Shores outpatient services, using standardized screening tools and protocols

  • Conduct brief triage interviews to understand client needs and match them to appropriate programs.

  • Support clients in completing pre-screening measures, consent forms, and enrollment into patient portals

  • Redirect referrals outside eligibility criteria to appropriate resources (e.g., based on geography or age)

  • Maintains meeting established volume targets


Care Coordination & System Navigation



  • Provide practical assistance related to social determinants of health (housing, finances, education, employment) to help remove barriers to care

  • Facilitate communication with primary care providers, referral sources, and partner agencies to ensure continuity of care and smooth transitions

  • Connect clients and family members to waitlist supports (self-help, online tools), and crisis resources as appropriate


 Partnership Development & Stakeholder Engagement



  • Foster positive relationships with referral sources, community partners, and service providers across the East Region

  • Participate in team meetings and quality improvement activities to enhance service delivery and coordinated access


Data, Reporting & Program Support



  • Accurately document all screening, triage, and navigation activities within the coordinated access technology systems

  • Collect data to support performance measurement, evaluation, and continuous quality improvement initiatives

  • Participate in research and program development initiatives that strengthen coordinated access and client outcomes


 


Health & Safety



  • Work in compliance with all health and safety regulations, including organizational standards and policies

  • Recognize and report client, staff, and environmental safety issues and participate in implementing corrective actions


 


Qualifications



  • Education: College diploma or Bachelor’s degree in Nursing, Social Work, Psychology, Health Sciences, Behavioural Sciences, or related field

  • Experience: Minimum 3–5 years’ experience in mental health and/or addictions sector, including intake, navigation, or case management

  • Knowledge & Skills:


    • Familiarity with stepped-care and coordinated access models.

    • Knowledge of mental health and substance use conditions, evidence-based interventions, and community resources.

    • Strong assessment, triage, and service planning skills.

    • Excellent interpersonal, communication, and documentation abilities.

    • Ability to work independently and collaboratively within a multidisciplinary and virtual team environment.

    • Proficiency in electronic health record systems and Microsoft Office Suite


  • Ability to manage competing priorities and work under pressure

  • Other Requirements: CPR/BLS, valid driver’s license, and ability to travel within the region.


 


Freedom of Action



  • Works independently and interdependently within a multidisciplinary team, following program standards and organizational policies

  • Escalates complex cases and safety concerns to the Clinical Manager as needed

  • Work is monitored through documentation review, regular supervision, and performance discussions
