Your Opportunity:
As a Pharmacist 1, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will be responsible for direct patient care services both on clinical coverage and as part of the dispensary team (order verification), education/research activities, and providing drug information to pharmacy, nursing, medicine & other members of the multidisciplinary team as required. Pharmacists responsible for clinical coverage actively participate in patient care rounds, collaborate with members of the health care team & develop professional relationships with patients & families to formulate and implement a medication treatment plan. Other roles include precepting & teaching of pharmacy residents, staff, and students.
Description:
As a Pharmacist I, you will be responsible for the safe, effective and efficient preparation and distribution of pharmaceutical services. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging.
Required Qualifications:Bachelor of Science in Pharmacy. Active or eligible for registration with the Alberta College of Pharmacy (ACP).
Additional Required Qualifications:Advanced Prescribing Authority (APA).Successful completion of PEBC (or pending results).Recent experience as a pharmacist in a hospital setting providing comprehensive medication management services in a collaborative care model.Excellent oral and written communication and interpersonal skills.Proven organizational and decision-making abilities.Demonstrates ability to problem solve and use evidence-based medicine in direct patient care.Demonstrates self-directed learning, initiative and continuing professional development.Computer literacy for drug information and work processing.Ability to work with a wide variety of people in health care environment with professionalism and respect.Ability to utilize critical thinking skills and work independently when required.Personal and professional accountability.
Preferred Qualifications:Completion of a Canadian accredited pharmacy residency program or advanced clinical pharmacy degree (ex. post-baccalaureate PharmD).Recent project experience, volunteer experience or committee work.Recent experience with training and precepting.Active membership in the Canadian Society of Hospital Pharmacists.
