Chief Kinosew Awasak Mikwahp (Driftpile Children’s Lodge) is a community-based and family-oriented organization dedicated to providing care, protection, and well-being for all Driftpile Cree Nation children in a preventative manner, promoting healing for children, families, and the community. A coordinated, holistic, and interdisciplinary approach forms the foundation of the Agency, ensuring that the developmental needs of children, youth, and families are met with respect for their cultural identity, customs, traditions, beliefs, and values.
JOB OVERVIEW
Post Majority offers individualized support services to youth leaving care and young adults formerly in care aged 15-26 as they prepare for independence and are exiting or leaving care due to having reach the age of majority when mandated or legislated services end and for those young adults formerly in care.
Reporting to the PM Supervisor, the PM Worker is responsible for engaging youth and young adults to increase their access to timely and effective services with clear pathways to access care, shorter wait times, coordinated and wraparound service delivery, including services that are youth friendly. The incumbent will work with Driftpile. The PM Worker also provides young people with identified needs and self-identified goals, opportunities to address basic needs through individualized life plans. The goal is to provide comprehensive wraparound services that promote, empower and enable young people to achieve personal independence. This position is responsible to manage case assignments, review and follow up, support and advocate to meet the needs of young people. They will help evaluate, develop individual life plans, and collaborate and liaise with program and service providers to help and advocate for youth and young adults in the PM program.
DUTIES & RESPONSIBILITIES
Knowledge of the Post Majority program policies and practices.
Conduct initial and comprehensive assessments of identified needs, support systems, and develop individualized life plans identified by the young adult ensuring ongoing case management and follow up.
Assist youth and young adults in creating individualized life plans and self-identified goals addressing basic needs. This may include assistance with housing, food, employment and financial security, mental health, wellness, addiction supports, cultural supports, life skills, healing relationships and connecting with supportive family and community.
Monitor progress against expected and known outcomes, maintain accurate records and follow up.
Active understanding of young adult developmental stages, trauma informed practice including intergenerational trauma and long-term effects of trauma experienced while in care.
Assist young people in navigating through systems (book appointments, provide transportation, assist with forms and applications, etc.).
Provide support services and advocacy from a trauma informed lens.
Focus on referral services for at-risk young people that includes encouragement and care by tapping into required programs and services.
Strong knowledge of community resources including but not limited to social, legal, health and financial (internal and external resources).
Establish networks and collaborative environments that include the view of young people that are incorporated in all processes and requests.
Maintain working relationships and coordinate/navigate programs and services with team colleagues, partnering agencies and other service providers.
Integrate multiple party feedback and maintain excellent communication. Self-reflection, sharing, and improvement as an individual and as part of a larger team is key.
Anticipate barriers and solutions to overcome systemic issues and gaps.
Maintain confidentiality, discretion, and ethical practice.
Monitor, assess, review caseload and file information. Maintain working files that are accurate, concise and current.
Provide Supervisor with timely briefings and information, enabling informed decision-making, including reporting and data collection methods.
Perform other related duties, as requested.
QUALIFICATIONS
Diploma or Degree in Human Services, Social Work, or related field.
Minimum of three (3) years experience in a related position.
Valid Class 5 Driver’s License, with an up-to-date driver’s abstract.
Clear Criminal Record and Child Intervention Record checks.
Microsoft Office software, emails, databases, excel, PowerPoint, and outlook.
Knowledge of First Nation culture and communities.
Cree language is an asset.
