ABOUT THE ROLE


Position: Program Facilitator, Addictions (Back on Track Facilitator)


Type of role: casual (weekly hours vary)


Location:
Remote and at ADAPT offices (Burlington or Oakville), if needed

WHAT YOU'LL DO


On a fee for delivery basis, Back on Track (BOT) Facilitator provides group facilitation and counselling services to participants of the ADAPT BOT program within the parameters outlined in the ADAPT BOT Operations Manual and as defined by the Ministry of Transportation of Ontario (MTO) and the service contract with the Centre for Addiction and Mental Health (CAMH).



  • conducts Back on Track group sessions as required, specifically the one-day “Education” and/or the two-day “Treatment” Group Sessions, as scheduled by the Back on Track Program Coordinator and according to the procedures and standards set by ADAPT, the MTO and CAMH

  • registers participants for all appropriate Back on Track services

  • conducts Back on Track assessments, when scheduled, according to established procedures and standards

  • assigns participants to either an education group or a treatment group according to the results of assessments

  • conducts Back on Track follow up according to established procedures and standards

  • provides consultative services to community agencies, professionals or individuals as required

  • maintains accurate and complete participant records according to guidelines established by the MTO and CAMH

  • provides all required information to “contact for miscellaneous” as required

  • completes Serious Incident Reports and submits “medical review” information according to established procedures and standards

  • participates in training sessions as required

  • provides input into the operational and strategic planning related to ADAPT’s Back on Track program as required

  • assists in maintaining a high level of open and effective communications and co-operation within the organization

  • provides other services according to the conditions of program funding

  • may participate in committees as required

  • participates in community education and public awareness as required



WHAT WE ASK FOR



    Education


    University degree/ diploma in Social Work/ Social Service and/ or Health related field plus Addiction Studies certification, or equivalent combination of experience and education



    Licenses, certification, credentials



    • Must complete a security clearance, as required by the Ministry of Transportation-Ontario

    • Non-violent crisis intervention, First Aid, CPR and ASIST certifications are assets;



    Experience



    • 3 years of counselling experience in addictions or a related field;

    • Experience facilitating online learning;



    YOUR SKILLS AND KNOWLEDGE



    •  Ability to be flexible with hours of work to accommodate participants not available during conventional business hours

    • Knowledge of privacy legislation and practices

    • Knowledge of the law pertaining to impaired driving offenses is an asset

    • Knowledge of addictions treatments and explanatory theories (with an emphasis on harm-reduction), mental health, concurrent disorders and community resources is required

    • Familiarity with community services and resources

    • Strong communication and active listening skills

    • Attention to detail

    • Ability to: function independently, maintain effective working relationships with others, work effectively under pressure or deadlines and work outside regular office hours as required, use good judgement to make sound decisions

    • Interpersonal and empathy skills facilitating rapport building with clients

    • Ability to maintain consistency with program delivery standards

    • Experienced sense and awareness of ethical boundaries and clinical reporting obligations

    • Good organizational and multitasking skills

    • Active time management and scheduling

    • Ability to effectively present complex clinical information in writing and verbally

    • Ability to successfully deliver culturally responsive services

    • Cultural knowledge of key populations in Halton Region that will be served to address disparities in specific services

    • Proficiency with computer programs and electronic database(s)



    WORK CONDITIONS


    Work conditions depend on the ADAPT model of operations, and may, but do not have to, include a combination of remote and in-person work arrangement



    • Working remotely from home-office

    • Working in a busy office environment with frequent interruptions;

    • Flexible hours, may include weekends;

    • Manual dexterity required to use desktop computer and peripherals.

    • Various physical actions of mobility are required (minimal lifting, sitting for periods of time, moderate wrist motions).
