Program Manager - Child, Youth, and Adult, Specialized Homes - Full Time


The Program Manager will assist the Services Management Team and especially the Case Management Supervisor in different aspects of Case Management and particularly supervising Case Managers to achieve their goals.


Education & Experience



  • Three years of recent related experience, including 1 year of supervisory or administrative experience.

  • University bachelor’s degree/diploma in a related human/social service field or equivalent.

  • Specific training in behavioural modification of children and youth under 19 years of age with support needs, attachment-based parenting, and relationship-based interventions (Trauma-informed practice).

  • Physically fit and in excellent health.

  • Valid class 5 driver's license with no restrictions and a good driving record.

  • No criminal convictions or criminal charges pending.

  • No history of improper conduct in all areas.

  • Fluent in both written and verbal English.

  • Holds First Aid certificate.

  • Excellent interpersonal communication skills are required to deal with residents, family members, staff, and professional medical associates.

  • Must have demonstrated management skills.

  • Having at least 2 years of support worker experience is an asset.


Requirements



  • Oversees development of the Service Plan by the case management team (Care Circle) and in collaboration with the individual in care.

  • Assists Case Management Supervisor in overseeing the day-to-day operations of the worksites and provides ongoing supervision of programs to ensure an integrated and holistic assessment and planning is in place.

  • Assists the Services and Quality Assurance Management teams to evaluate program goals, objectives, policies and procedures, and ensures the required standards are maintained. Identifies both physical and program needs of the individuals to report to appropriate authority.

  • Supports Case Management team and Subcontractors with implementation of Strategies established in Individual Service Plans.

  • Leads development of the individual’s transition plan, drawing on other experts where necessary.

  • Provides leadership and guidance to Case Managers and staff, Subcontractors, families, and others in the preparation of related documents and reports.

  • Oversees training of staff members and subcontractors by Case Managers.

  • Facilitates access to supplemental services to support individuals, their families, staff, and Subcontractors.

  • Liaises with the community, government, families, officials, and professionals and promotes community involvement in the program.

  • Assists in recruiting and selecting support workers

  • Assists in the orientation of new employees and provides guidance, training and orientation on policies, procedures, techniques, report preparation or other matters arising in the work site.

  • Identifies case managers' needs for professional development.

  • Assists the service management in making Quality Improvement Plans, progressive disciplines, letters of expectations, etc. for case managers.

  • Oversees the cleanliness, safety, security, and maintenance of the work site in accordance with applicable licensing standards either directly or through delegation to case managers.

  • Reports to the Case Management Supervisor daily regarding any important updates about the program, staff, individuals in care, and critical incidents.

  • Supports case managers (residence coordinators) with the finalization of Critical Incidents reporting in a timely manner (within 24 hours of the incident). Supports case manager in debriefing staff after an incident, as needed.

  • Assists Case Management Supervisor in supervising case managers’ Schedules and Appointments, in completing and following-up Services Inspection Checklist.

  • Supports case managers for Meetings with CBI, social workers, doctors, and school officials.

  • Oversees case manager reporting on health appointments, and health and wellbeing of person in care.

  • Ensures that Monthly/ Quarterly reports are proofread, completed, and submitted by the end of every month/quarter to the authorities.

  • Supervises case managers during house inspections by the funders and ensures all requirements are in place.

  • Assists Case Management Supervisor with case manager's annual performance evaluation and makes necessary recommendations.

  • Updates case managers on new company policies, information, protocol, guidelines, memos, announcements, etc.

  • Provides training to case managers and updating programs as needed.

  • Assists Case Management Supervisor in setting up regular case managers meetings and securing the minutes.

  • Fulfills other tasks relevant to the job description that are not listed above but assigned by the Case Management Supervisor or Services Manager

Only Full-Time, Contract-base


Contract length: 12 months, with the possibility of renewal on an annual basis (long-term) based on performance and company needs


Pay Grade


Starting salary: $35.00 per hour during probation which is 6 month and $37 after successfully passing the probation (years of experience and previous positions may affect the starting salary)


Schedule



  • 8-hour shift weekdays (Mon- Fri , 9a-5p)

  • Weekend availability (occasionally if demanded)

Work Location


Head office is in Vancouver (Ontario street) but the position may require daily travelling in Lower mainland area including Vancouver, Richmond, Tri-cities, New Westminster, Delta, and Surrey for monitoring and other purposes. Vehicle will be provided by the company.
