Psychogeriatric Resource Consultant (PRC) 

1  Permanent Part-time – Sarnia Site

Reporting to the Manager of Integrated Client Services, the incumbent will be responsible for the following functions:

• Providing geriatric specific assessments and treatment planning.


• Liaising, when appropriate, with other care givers/support providers who are involved to ensure continuity of care.


• Supporting the implementation of the treatment plan.


• Advocates for older adults and minimizes the risk associated with responsive behaviours.


• Provides clinical leadership to the Geriatric Mental Health Outreach Team, BSO lead and in house teams as needed.


• Collaborates with agencies that provide healthcare, community and social services for older adults.


• Identifies potential risks in meeting targets/metrics specific to BSO and collaborates with the Regional Coordinator for solutions and/or advocates for Governance level intervention.

Qualifications:   

• Bachelor of Science in Nursing degree and Registered with the College of Nurses of Ontario.


• Experience and detailed knowledge of serious mental illness, dementia, and other age-related conditions of older adults usually gained by 3 years of experience in working with this population.


• Valid Ontario Drivers License and access to a vehicle.
  

Hours of Work:   28 hours/week. Flexible hours are required to meet the position requirements.

 Salary: Commensurate with experience and education within the range: $72,405 - $88,092, inclusive of HOOPP.
