Job Description: The MLTC Health & Social Development Department is seeking highly motivated individuals to fill full-time, permanent positions of Recovery Outreach Specialists. The Recovery Outreach Specialists report to the Addiction Medicine Program Lead and work as an integral member of the MLTC Addiction Medicine Program.
The Recovery Outreach Specialists provide holistic, community-based nursing care within MLTC communities to support individuals living with substance use disorders, including those with concurrent mental health conditions and co-occurring communicable or chronic diseases. This position plays a key role in delivering comprehensive addiction medicine services, including opioid agonist therapy (OAT), harm reduction initiatives, and strengthened continuity of STBBI care.
This role bridges clinical nursing practice and community-based outreach, prioritizing flexible, relationship-based care for individuals experiencing barriers to traditional health services. Care is delivered in a culturally safe, trauma-informed, and recovery-oriented manner, grounded in dignity, respect, and hope.
