About the Program


Foundry is an integrated youth services (IYS) initiative for youth ages 12-24 throughout British Columbia (BC). Foundry removes barriers and improves care pathways for youth by supporting co-location and integration of existing resources and services for five core streams: physical and sexual health, mental health, substance use, peer support and social services. Located in Surrey City Centre, Foundry Surrey Central serves a diverse and growing community and is committed to accessible, culturally safer, youth- and family-centred care.


Position Summary


PCRS is seeking a Masters-Level Registered Social Worker to join the team at Foundry Surrey Central. Working within a collaborative interdisciplinary service model, the successful candidate will provide clinical social work services to youth ages 12–24 and their families/caregivers, including psychosocial assessment, therapeutic intervention, crisis response, case formulation, care planning, and service coordination within an integrated youth services setting.


Responsibilities



  • Provides clinical social work assessment and treatment services to youth ages 12–24 and their families/caregivers, including individual, family, and group interventions grounded in trauma-informed, developmentally responsive, and culturally safer practice

  • Conducts comprehensive biopsychosocial, risk, and safety assessments and develop collaborative service plans informed by clinical formulation, youth strengths, identified needs, and service goals

  • Provides crisis intervention, suicide risk assessment, safety planning, and brief therapeutic response in situations involving acute distress, mental health concerns, or psychosocial instability

  • Provides therapeutic support to youth presenting with mental health, substance use, trauma-related, and concurrent concerns using evidence-informed and relational approaches

  • Collaborates with interdisciplinary team members, primary care providers, and community partners to support coordinated, holistic, and youth-centred service delivery

  • Provides clinical bridging and coordination between primary health care and other Foundry services to support integrated assessment, shared case formulation, collaborative care planning, continuity of care, and seamless service transitions

  • Provides case management and service navigation for youth with complex, concurrent, or intersecting needs, including coordination of care, systems navigation, advocacy, follow-up, and facilitation of access to appropriate internal and external services and resources

  • Facilitates referrals and ongoing care coordination with internal and external supports, including mental health, substance use, primary care, housing, education, employment, and community-based services

  • Maintains accurate, timely, and professional clinical documentation in accordance with agency policy, applicable legislation, ethical standards, and regulatory requirements

  • Participates in case consultation, interdisciplinary case conferencing, service planning, and continuous quality improvement activities

  • Contributes to a safe, welcoming, and youth-friendly service environment that reflects principles of equity, inclusion, and relational practice

  • Establishes and maintains effective therapeutic and collaborative relationships with youth, families/caregivers, colleagues, community partners, and external stakeholders

  • Performs other related duties as required.


Competencies & Knowledge



  • Demonstrated knowledge of adolescent and emerging adult development, mental health, trauma, and the social determinants of health

  • Training and experience in evidence-informed therapeutic modalities such as CBT, DBT, Solution-Focused Therapy, Narrative Therapy, or other relevant clinical approaches

  • Knowledge of the Foundry integrated youth services model and commitment to low-barrier, youth-centred, and relational service delivery

  • Understanding of the strengths, needs, and systemic barriers experienced by Surrey’s diverse communities

  • Demonstrated ability to engage youth and families from a broad range of cultural, linguistic, and lived-experience backgrounds

  • Commitment to collaborative, relationship-based, and family-inclusive social work practice

  • Clear and concise communication skills, both verbal and written

  • Sound judgment, tact, and effective interpersonal skills

  • Demonstrated knowledge and application of trauma-informed and culturally safe principles of care

  • Ability to work effectively with others in a multicultural, interdisciplinary environment, independently and as part of an interprofessional team, including understanding the frameworks of other disciplines

  • Strong crisis management skills

  • Able to identify and respond appropriately to legal and ethical issues that may arise in client care

  • Confidence in making effective referrals in a collaborative manner, reflecting in-depth knowledge of community resources and strong collaborative skills in working with other community partners

  • Ability to exercise sound clinical judgment, professional boundaries, and flexibility in a dynamic service environment

  • Physical ability to carry out the duties of the position
