Job Description - Registered Social Worker or Psychotherapist
The Centre for Family Medicine (CFFM) Family Health Team is currently seeking a 0.7 FTE Permanent Registered Social Worker or Psychotherapist.
The Centre for Family Medicine (CFFM) has a mission "To provide leadership and excellence in comprehensive patient-centred primary care through education, research, innovation and care provision in a collaborative, sustainable, interprofessional environment".
Our team currently consists of nurse practitioners, registered practical nurses, registered nurses, 24 family physicians, part time specialist physicians, social workers and psychotherapists with additional training in couple and family therapy, dietitians, and clinical pharmacists.
Position Overview
The CFFM is hiring a Social Worker or Psychotherapist as part of a new program to increase patient access to mental health services in KW4 subregion. Reporting to the Manager of the Mental Health program, the clinical therapist will provide direct care to patients at CFFM partner organizations, including a family health organization.
Responsibilities
The successful candidate will be responsible for liaising with physicians, conducting on-site visits to their offices, and cultivating strong professional relationships with the health partnered organizations
Collaborate with physicians and team members at the health partner organization(s) to collectively manage the needs of the patients
Provide counselling, case management and support to patients of the health partner organization.
Collaborate with community resource organizations as needed
Communicate effectively in written and verbal form on behalf of patients. This includes communicating with primary care providers as well as charting case notes in an EMR
Provide short term counselling services to patients referred by family physicians within the partnered organization
Facilitate mental health related groups and workshops, as required
Proficient in applying a variety of theoretical models and perspectives to treat patient's presenting concerns.
Effectively complete assessment, treatment and management of a wide range of mental health issues
Collaborate with physicians and team members to collectively manage the needs of patients
Additional duties that arise as the role and relationship with the Family Health Organizations develop
Skills & Qualifications
Masters degree in Social Work or Psychotherapy from a registered educational institution
Registration with applicable College and proven evidence of liability insurance
Strong experience in delivering clinical counselling/therapy, preferably in a community setting
Experience or strong interest in working with marginalized populations, patients with addictions, trauma histories and mental health diagnoses
Excellent problem solving and interpersonal skills
Excellent verbal and written communication skills
Demonstrated ability to work within teams
Demonstrated ability to work with ambiguity and minimal supervision
