Registered Social Worker or Psychotherapist
The Centre for Family Medicine (CFFM) has a mission “To provide leadership and excellence in
comprehensive patient-centred primary care through education, research, innovation and care
provision in a collaborative, sustainable, interprofessional environment.”
Our team currently consists of nurse practitioners, nurse educators, registered practical nurses, registered nurses, twenty-four family physicians, part time specialist physicians, social workers and psychotherapists with additional training in couple and family therapy, dietitians, and clinical pharmacists.
Position Overview
The Social Worker/Psychotherapist will provide comprehensive and coordinated mental health services to Centre for Family Medicine rostered patients and will function in close collaboration with physicians and other inter-professional health care providers within the Family Health Team.
As a vital member of our patient focused team, the social worker/psychotherapist will provide clinical services and will practice to their full scope of practice as defined by their regulatory College.
Responsibilities
Provide short term counseling services to patients referred by family physicians
Experience working with individuals 12 years of age and older, couples and families
Facilitate mental health related groups and workshops, as required
Proficient in applying a variety of theoretical models and perspectives to treat patient’s presenting concerns
Effectively complete assessment, treatment and management of a wide range of mental health issues
Make referrals to and liaise with other professionals on the inter-disciplinary team and within the community
Collaborate with physicians and team members to collectively manage the needs of patients.
Collaborate with community resource organizations as needed.
Communicate effectively in written and verbal form on behalf of patients. This includes communicating with primary health care providers, other team members and community care providers as well as charting case notes in EMR.

Skills & Qualifications
Masters degree in Social Work or Psychotherapy from a registered educational institution
Registration with applicable College and proven evidence of liability insurance
Strong experience in delivering clinical counselling / therapy, preferably in a community setting
Experience or strong interest in working with marginalized populations, patients with addictions, trauma histories and mental health diagnoses
Excellent problem solving and interpersonal skills
Excellent verbal and written communication skills
Demonstrated ability to work within teams
Demonstrated ability to work with ambiguity and minimal supervision
