This position is posted by Jobgether on behalf of a partner company. We are currently looking for a Remote Nurse Case Manager. In this role, you will play a crucial role in supporting patients through their medical journeys, ensuring they receive the necessary care and resources. You will manage a diverse caseload, collaborate with healthcare providers, and lead training initiatives for team members. This position allows you to make a significant impact in the healthcare sector while working remotely, providing flexibility and the opportunity to enhance the lives of patients and their families.
Accountabilities
- Responsible for assigned caseload and meeting inbound/outbound call targets
- Manage reimbursement and appointment coordination for patients
- Provide clinical support and guidance on the enrolment process
- Review feasibility of requests for medication administration
- Ensure accurate and up to date documentation for patient appointments
- Lead disease-state training of new/existing team members
- Provide feedback on protocols and inter-departmental relationships
- Report all Adverse Events to the assigned units/departments
Requirements
- Registered Nurse (RN) or Registered Practical Nurse (RPN) diploma with five (5) years of Nursing experience
- Healthcare and case management experience preferred
- Bilingualism (French and English) strongly preferred
- Experience in oncology is an asset
- Strong patient focus and critical thinking skills
- Effective communication skills, both orally and in writing
- Ability to multi-task and work under pressure
- Proficiency in Microsoft Office programs
